香砂六君子汤联合美利曲辛治疗脾胃气虚型失眠症的效果  

Effect of Xiangsha Liujunzi Decoction Combined with Melitracen in the Treatment of Insomnia Due to Spleen and Stomach Qi Deficiency

摘　　要：目的观察香砂六君子联合氟哌噻吨美利曲辛片治疗脾胃气虚型失眠症的效果。方法选取2022年1月-2024年1月于苏州市中医医院收治的失眠症患者102例,随机分为联合组(n=51)和对照组(n=51)。对照组予以口服氟哌噻吨美利曲辛片,1片/次,早晨、中午各1次。联合组在对照组基础上加用香砂六君子汤,2次/d,1剂/d。所有患者均连续治疗8周。记录并比较两组患者治疗前及治疗4周、8周后的中医证候积分、匹兹堡睡眠质量指数评分表(PSQI)评分以及血清多巴胺(DA)、5-羟色胺(5-HT)、胃泌素(GAS)、血管活性肠肽(VIP)水平,比较两组患者不良反应发生情况。结果联合组总有效率高于对照组(χ^(2)=6.331,P<0.05)。治疗4周、8周后两组中医证候积分、PSQI评分、血清DA、VIP低于治疗前(P<0.05),且联合组低于对照组(P<0.05);血清5-HT、GAS水平高于治疗前(P<0.05),且联合组高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(χ^(2)=1.562,P>0.05)。结论香砂六君子汤联合美利曲辛能改善脾胃气虚型失眠症患者的临床症状,提升患者的睡眠质量,有效调节血清神经递质和胃肠激素水平,安全性较好。Objective To observe the effect of Xiangsha Liujunzi combined with flupentixol-melitracen tablets in the treatment of insomnia due to spleen and stomach Qi deficiency.Methods 102 patients with insomnia admitted to our hospital from January 2022 to January 2024 were selected and randomly divided into a combined group(n=51)and a control group(n=51).The control group was given flupentixol-melitracen tablets orally,1 tablet/time,once in the morning and once at noon.The combined group was given Xiangsha Liujunzi Decoction,2 times/d,1 dose/d.All patients were treated continuously for 8 weeks.The TCM evidence points,Pittsburgh Sleep Quality Index(PSQI)scores,and serum dopamine(DA),5-hydroxytryptamine(5-HT),gastrin(GAS),and vasoactive intestinal peptide(VIP)levels of the two groups were recorded before and after 4 and 8 weeks of treatment,and the incidence of adverse reactions was compared between the two groups.Results The total effective rate of the combined group was higher than that of the control group(χ^(2)=6.331,P<0.05).After 4 weeks and 8 weeks of treatment,the TCM syndrome scores,PSQI scores,serum DA,and VIP of the two groups were lower than before treatment(P<0.05),and the combined group was lower than those of the control group(P<0.05).Serum 5-HT and GAS levels were higher than before treatment(P<0.05),and the combined group was higher than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(χ^(2)=1.562,P>0.05).Conclusion Xiangsha Liujunzi Decoction combined with Melitracen can improve the clinical symptoms of patients with insomnia due to spleen and stomach qi deficiency,improve the patient’s sleep quality,effectively regulate serum neurotransmitters and gastrointestinal hormone levels,and has good safety.
