摘 要:目的:对比膝关节滑膜炎患者应用脂肪抑制液体衰减反转恢复MRI(Fluid-attenuated Inversion Recovery-Fat Suppressing-MRI,FLAIR-FS-MRI)和增强MRI成像的诊断效能。方法:选取2022年6月至2024年1月我院接收的98例疑似膝关节滑膜炎患者,均进行病理检查、FLAIR-FS-MRI成像与增强MRI成像检查。以病理检查为金标准,分析FLAIR-FS-MRI成像与增强MRI成像检查的诊断结果,并对比两者效能,滑膜厚度分级和滑膜病变。结果:FLAIR-FS-MRI、增强MRI成像诊断膝关节滑膜炎患者结果均为阳性83例、阴性15例。FLAIR-FS-MRI成像诊断膝关节滑膜炎患者的灵敏度、特异度、阳性率、阴性率、准确率均较增强MRI成像更高(P<0.05)。FLAIR-FS-MRI、增强MRI成像诊断膝关节滑膜炎患者的滑膜厚度分级比较差异无统计学意义。FLAIR-FS-MRI成像诊断膝关节滑膜炎患者的关节腔滑膜积液厚度、周围软骨病变和关节间隙变窄例数明高于增强MRI成像(P<0.05)。结论:与增强MRI成像相比,在诊断膝关节滑膜炎患者中应用FLAIR-FS-MRI成像诊断效能更佳,且诊断膝关节滑膜炎患者的关节腔滑膜积液厚度、周围软骨病变和关节间隙变窄例数更高。Objective:To compare the diagnostic efficacy of Fluid-attenuated Inversion Recovery-Fat Suppressing-MRI(FLAIR-FS-MRI)and enhanced MRI in patients with knee synovitis.Method:Pathological examination,FLAIR-FS-MRI and enhanced MRI were performed in 98 patients with suspected knee synovitis admitted to our hospital from June,2022 to January,2024.Using pathological examination as the gold standard,the diagnostic results of FLAIR-FS-MRI and enhanced MRI were analyzed,and the efficacy,synovial thickness grade and synovial lesions of the two were compared.Results:The results of FLAIR-FS-MRI and enhanced MRI in diagnosis of knee synovitis were positive in 83 cases and negative in 15 cases.The sensitivity,specificity,positive rate,negative rate and accuracy of FLAIR-FS-MRI in diagnosing knee synovitis were higher than those of enhanced MRI(P<0.05).There was no significant difference in the grade of synovial thickness between FLAIR-FS-MRI and enhanced MRI in the diagnosis of knee synovitis.The number of cases of synovial effusion thickness,peripheral cartilage lesion and joint space narrowing in patients with knee synovitis diagnosed by FLAIR-FS-MRI was higher than that by enhanced MRI(P<0.05).Conclusion:Compared with enhanced MRI,FLAIR-FS-MRI was more effective in diagnosing synovitis of knee joint,and more cases of synovial effusion thickness,peripheral cartilage lesions,and joint space narrowing were diagnosed in patients with synovitis of knee joint.
