摘 要:目的:探究非优质与优质卵裂期胚胎囊胚培养后行冻融周期单囊胚移植的结果。方法:选取2021年1月-2023年7月期间在本院生殖科接受冻融囊胚移植的7177枚囊胚作为研究对象,根据卵裂期胚胎的细胞数量和碎片率分为优质组(优质卵裂期胚胎,n=4578)与非优质组(非优质卵裂期胚胎,n=2599)。分析对比两组囊胚形成率。随访两年,统计两组在随访期间的妊娠情况,包括临床妊娠率、流产率、活产率、多胎妊娠率以及胎儿畸形率。结果:非优质组移植D5可利用囊胚率、移植D6可利用囊胚率、移植D7可利用囊胚率均显著高于优质组;非优质组移植D5优质囊胚率、移植D6优质囊胚率均显著低于优质组(P<0.05)。非优质组的临床妊娠率、活产率显著低于优质组(P<0.05)。结论:非优质卵裂期胚胎囊胚培养后行冻融周期单囊胚移植可提高胚胎利用率、降低多胎妊娠风险,为临床辅助生殖技术提供新的选择。Objective:To investigate the results of freeze-thaw cycle single blastocyst transfer after blastocyst culture of non-high quality and high quality cleavage stage embryos.Methods:A total of 7177 blastocysts that received freeze-thawed blastocyst transfer in the reproductive Department of our hospital from January 2021 to July 2023 were selected as research objects.According to the number of cells and fragmentation rate of embryos at cleavage stage,they were divided into high quality group(high quality embryos at cleavage stage,n=4578)and non-high quality group(non-high quality embryos at cleavage stage,n=2599).The blastocyst formation rates of the two groups were analyzed.The two groups were followed up for two years,and the pregnancy status of the two groups during the follow-up period was analyzed,including clinical pregnancy rate,abortion rate,live birth rate,multiple pregnancy rate and fetal malformation rate.Results:The rates of transplanted D5,transplanted D6 and transplanted D7 available blastocysts in non-quality group were significantly higher than those in quality group.The rates of D5 and D6 blastocysts transplanted in non-quality group were significantly lower than those in quality group(P<0.05).The clinical pregnancy and live birth rate of non-quality group was significantly lower than that of quality group(P<0.05).Conclusion:Single blastocyst transfer during freeze-thaw cycle after blastocyst culture can improve embryo utilization rate,reduce the risk of multiple pregnancy,and provide a new choice for clinical assisted reproductive technology.
