摘 要:目的:探讨小儿过敏性紫癜(Henoch Schoenlein purpura,HSP)患儿发生急性肾损伤状况及其影响因素。方法:回顾性收集2019年1月-2023年12月信阳市人民医院收治的103例HSP患儿临床资料。统计HSP患儿急性肾损伤发生情况,并根据HSP患儿急性肾损伤的发生情况,将患儿分为发生组和未发生组。收集并对比两组患儿的基线资料。多元Logistic回归分析检验HSP患儿发生急性肾损伤的影响因素。结果:103位HSP患儿中有15例发生急性肾损伤,发生率为14.56%,纳入发生组。88例HSP患儿未发生急性肾损伤,纳入未发生组。发生组的中性粒细胞/淋巴细胞比值(Neutrophil to Lymphocyte Ratio,NLR)、血小板/淋巴细胞比值(Platelet to lymphocyte ratio,PLR)水平、维生素D水平与未发生组均有显著差异(P<0.05)。两组的年龄、性别、HSP类型、服用皮质类固醇、血抗链球菌溶血素O性质、静脉注射免疫球蛋白、24 h尿微量白蛋白排泄率、尿素氮等其他资料均无显著差异(P>0.05)。多元Logistic回归分析,结果显示,NLR水平高、PLR水平高、维生素D水平低是HSP患儿发生急性肾损伤的影响因素(OR>1,P<0.05)。结论:HSP患儿急性肾损伤发生率略高,NLR水平高、PLR水平高、维生素D水平低是HSP患儿发生急性肾损伤的影响因素。Objective:To investigate the status and influencing factors of acute kidney injury in children with Henoch Schoenlein purpura(HSP).Methods:Clinical data of 103 children with HSP treated in Xinyang People's Hospital from January 2019 to December 2023 were retrospectively collected.The incidence of acute kidney injury in children with HSP was analyzed.According to the incidence of acute kidney injury in children with HSP,the children were divided into the occurrence group and the non-occurrence group.Baseline data were collected and compared between the two groups.Multivariate Logistic regression analysis was used to examine the factors affecting acute kidney injury in children with HSP.Results:Among 103 children with HSP,15 cases developed acute kidney injury(14.56%).88 children with HSP did not develop acute kidney injury and were included in the group without acute kidney injury.Neutrophil to Lymphocyte Ratio(NLR),Platelet to lymphocyte ratio(NLR),Platelet to lymphocyte ratio,There were significant differences in PLR and vitamin D levels between the two groups(P<0.05).There were no significant differences between the two groups in age,sex,HSP type,corticosteroid administration,blood antistreptococcal hemolysin O properties,intravenous immunoglobulin,urinary microalbumin excretion rate at 24 h,urea nitrogen and other data(P>0.05).Multivariate Logistic regression analysis showed that high level of NLR,high level of PLR and low level of vitamin D were the influential factors for acute kidney injury in children with HSP(OR>1,P<0.05).Conclusion:The incidence of acute kidney injury is slightly higher in children with HSP.High level of NLR,high level of PLR and low level of vitamin D are the influential factors for acute kidney injury in children with HSP.
