摘 要:目的:观察拉氧头孢联合阿奇霉素对小儿大叶性肺炎血清炎症因子及心肌酶谱的改善效果。方法:选取奉新县人民医院2021年1月至2024年7月收治的大叶性肺炎患儿60例,随机分为对照组和观察组(n=30)。对照组静脉滴注拉氧头孢40-80 mg·kg^(-1)治疗,观察组在对照组基础上口服阿奇霉素10 mg·kg^(-1)治疗,7 d后观察两组患儿临床疗效,比较两组治疗有效率和临床症状消退时间;分别于治疗前及7 d后采用酶联免疫吸附法测定C反应蛋白(C-reactive protein,CRP)、降钙素原(Procalcitonin,PCT)、白细胞介素6(Interleukin-6,IL-6)水平,采用生化酶速率法测定α-羟丁酸脱氢酶(α-hydroxybutyrate dehydrogenase,α-HBDH)、肌酸激酶(Creatine kinase,CK)、肌酸激酶同工酶(Creatine kinase isoenzyme MB,CK-MB)水平。结果:观察组患儿的治疗总有效率为93.33%,明显高于对照组的总有效率73.33%(P<0.05)。与对照组比较,观察组的发热、喘息、咳嗽、肺部湿啰音等临床症状消退时间均明显降低(P<0.05)。治疗后两组患儿血清CRP、PCT、IL-6水平均降低,且观察组上述血清指标水平低于对照组(P<0.05)。治疗后两组患儿α-HBDH、CK、CK-MB低于治疗前,且观察组各指标低于对照组(P<0.05)。两组不良反应总发生率分别为10.00%、23.33%,经比较无统计学差异(P>0.05)。结论:拉氧头孢联合阿奇霉素治疗小儿大叶性肺炎具有良好效果,值得推广。Objective:To observe the improvement effect of latamoxef combined with azithromycin on serum inflammatory factors and myocardial enzyme profile in children with lobar pneumonia.Methods:A total of 60 children with lobar pneumonia in Fengxin County People's Hospital were enrolled and randomly divided into the control group and the observation group between January 2021 and July 2024,30 cases in each group.The control group was treated with latamoxef(IV infusion,40-80 mg·kg^(-1)),while the observation group was treated with azithromycin(Orally,10 mg·kg^(-1))based on the control group.After 7 days of treatment,a clinical curative effect in the two groups was observed.C-reactive protein(CRP),procalcitonin(PCT),and interleukin-6(IL-6)levels were measured before treatment and 7 days later using enzyme-linked immunosorbent assay(ELISA).α-hydroxybutyrate dehydrogenase(α-HBDH),creatine kinase(CK),and creatine kinase isoenzyme MB(CK-MB)levels were determined using biochemical enzyme rate methods.Results:The total treatment response rate in the observation group was higher than that in the control group(93.33%vs 73.33%,P<0.05).The regression time of clinical symptoms(fever,wheezing,cough,lung moist rale)in the observation group was shorter than that in the control group(P<0.05).After treatment,serum CRP,PCT,and IL-6 levels decreased in both groups,which were lower in the observation group than in the control group(P<0.05).After treatment,the levels ofα-HBDH,CK,and CK-MB in both groups of children were lower than before treatment,and each indicator in the observation group was lower than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(10.00%vs 23.33%,P>0.05).Conclusion:Latamoxef combined with azithromycin shows good efficacy in the treatment of pediatric lobar pneumonia and is worth promoting.
