麦角新碱联合氨甲环酸对宫缩乏力性产后出血患者凝血功能及血清学指标的影响

摘　　要：目的:分析麦角新碱联合氨甲环酸对宫缩乏力性产后出血(PPH)患者凝血功能及血清学指标的影响。方法:选取2022年5月~2024年3月期间本院诊疗的宫缩乏力性PPH患者102例作为研究对象。根据治疗方法不同将患者分为对照组(氨甲环酸,52例)和观察组(麦角新碱联合氨甲环酸治疗,50例)。分析比较两组的临床疗效、不良反应发生率、产后出血量、止血时间、凝血功能[纤维蛋白原(FIB)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)]及血清学指标[白介素(IL)-6、促肾上腺皮质激素(ACTH)、肿瘤坏死因子(TNF)-α、皮质醇(Cor)]水平。结果:观察组的治疗总有效率显著高于对照组(P<0.05)。治疗后,观察组的产后出血量、止血时间降低幅度均显著大于对照组(P<0.05)。治疗后,两组的APTT、PT、TT水平均比治疗前显著降低,FIB水平均显著高于对照组;且观察组的APTT、PT、TT、FIB水平的变化幅度均显著大于对照组(P<0.05)。治疗后,两组的IL-6、ACTH、TNF-α、Cor水平均比治疗前显著提高,但观察组的IL-6、ACTH、TNF-α、Cor水平升高幅度均显著小于对照组(P<0.05)。两组的不良反应发生率无显著差异(P>0.05)。结论:麦角新碱联合氨甲环酸治疗宫缩乏力性PPH,能提高疗效,降低产后出血量,减少止血时间,改善凝血功能,减轻应激反应,且安全性较高。Objective:To analyze the effects of ergocline combined with tranexamic acid on coagulation function and serological indexes in patients with postpartum hemorrhage(PPH)caused by uterine fatigue.Methods:102 patients with uterine anemic PPH treated in our hospital from May 2022 to March 2024 were selected as the study objects.According to different treatment methods,the patients were divided into control group(tranexamic acid,52 cases)and observation group(ergocline combined tranexamic acid treatment,50 cases).The clinical efficacy,incidence of adverse reactions,postpartum blood loss,hemostasis time,coagulation function[fibrinogen(FIB),activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT)]and serological indexes[interleukin-6,adrenocorticotropin(ACTH),tumor necrosis factor(TNF)-α,cortisol(Cor)]levels of the two groups were analyzed and compared.Results:The total effective rate of observation group was significantly higher than that of control group(P<0.05).After treatment,the reduction of postpartum blood loss and hemostatic time in the observation group was significantly greater than that in the control group(P<0.05).After treatment,APTT,PT and TT levels in both groups were significantly lower than before treatment,and FIB levels were significantly higher than those in control group.The levels of APTT,PT,TT and FIB in the observation group were significantly higher than those in the control group(P<0.05).After treatment,the levels of IL-6,ACTH,TNF-αand Cor in both groups were significantly higher than those before treatment,but the levels of IL-6,ACTH,TNF-αand Cor in observation group were significantly lower than those in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:ergocline combined with tranatemocyclic acid in the treatment of uterine anemic PPH can improve the efficacy,reduce postpartum blood loss,reduce hemostatic time,improve coagulation function,relieve stress reaction,and have high s
