三维超声联合AMH检测在子宫切除患者卵巢储备功能评估中的价值分析

摘　　要：目的:探讨三维超声联合抗苗勒氏管激素(Anti-mullerian hormone,AMH)对子宫切除患者术后卵巢储备功能的评估价值。方法:选取2021年2月至2022年6月期间在本院完成子宫切除术治疗的128例患者作为研究对象。患者术前、术后7 d时均接受AMH及三维超声检查,获取超声检查参数[卵巢体积(Ovarian volume,OV)、卵巢间质动脉收缩期峰值流速(Peak systolic velocity of ovarian interstitial artery,PSV)、卵巢间质动脉血管化指数(Ovarian interstitial artery vascularization index,VI)]。术后1 m时检查性激素以及窦卵泡计数,诊断患者卵巢储备功能减退(Diminished ovarian reserve,DOR)发生情况,并将患者分为减退组和非减退组。分析比较两组术前、术后7 d时AMH及三维超声参数水平。分析超声检查主要参数对子宫切除患者术后DOR的评估价值。结果:两组术后7 d时PSV、VI及AMH均较术前降低,且减退组PSV、VI及AMH均明显低于非减退组(P<0.05)。绘制受试者工作特征曲线(Receiver Operating Characteristic curve,ROC),PSV、VI及AMH单独及联合评估子宫切除患者术后DOR的曲线下面积(Area under Curve,AUC)值均≥0.70,其中联合检查评估价值更高。结论:三维超声检查、AMH水平可从不同层面反映子宫切除患者卵巢储备功能,术后早期三维超声检查参数PSV、VI联合AMH可有效评估患者DOR发生情况。Objective:To evaluate the value of three-dimensional ultrasound combined with anti-Mullerian hormone(AMH)in ovarian reserve function after hysterectomy.Methods:128 patients who completed hysterectomy in our hospital from February 2021 to June 2022 were selected as the study objects.AMH and three-dimensional ultrasonography were performed on all patients before and 7 days after surgery,and ultrasound parameters[ovarian volume(OV),Peak systolic velocity of ovarian interstitial artery(PSV),and Ovarian interstitial artery vascularization index(VI)]were obtained.One month after surgery,diminished ovarian reserve(DOR)was diagnosed by examining sex hormones and sinus follicle count,and then the patients were divided into diminished and non-diminished groups.AMH and three-dimensional ultrasound parameters were analyzed and compared between the two groups before and 7 days after surgery.The value of main parameters of ultrasonography in the evaluation of postoperative DOR was analyzed in patients with hysterectomy.Results:PSV,VI and AMH in both groups on Day 7 after operation were significantly lower than those before operation,and PSV,VI and AMH in DOR group were significantly lower than those in non-DOR group(P<0.05).The receiver operating characteristic curve(ROC)was plotted,and the AUC value of postoperative DOR in PSV,VI and AMH alone and in combination were≥0.70,among which the combined examination had a higher value.Conclusion:Three-dimensional ultrasound and AMH levels can reflect the ovarian reserve function of patients with hysterectomy from different levels,and the early postoperative three-dimensional ultrasound parameters PSV and VI combined with AMH can effectively evaluate the occurrence of DOR in patients.
