苍附导痰汤辅助二甲双胍、炔雌醇环丙孕酮片治疗青春期肥胖型PCOS患者的临床疗效

摘　　要：目的:探讨苍附导痰汤辅助二甲双胍、炔雌醇环丙孕酮片治疗青春期肥胖型多囊卵巢综合征(Polycystic ovary syndrome,PCOS)的临床疗效。方法:选取我院2022年1月至2023年12月收治的青春期肥胖型PCOS患者108例,随机分为两组,每组54例。西药组给予二甲双胍、炔雌醇环丙孕酮片治疗,中药辅助组在此基础上给予苍附导痰汤治疗。治疗3 m后,比较两组临床疗效、肥胖指标体质指数(Body mass index,BMI)、腰臀比(Waist hips ratio,WHR),糖脂代谢相关指标空腹血糖(Fasting plasma glucose,FPG)、空腹胰岛素(Fasting insulin,FINS)、总胆固醇(Total cholesterol,TC)、甘油三酯(Triglyceride,TG),血清性激素促卵泡激素(Follicle-stimulating hormone,FSH)、黄体生成素(Luteinizing hormone,LH)、雌二醇、睾酮水平,病情相关因子血清瘦素、内脂素、胰岛素样生长因子结合蛋白-3(Insulin growth factors binding protein-3,IGFBP-3)、视黄醇结合蛋白4(Retinol binding protein-4,RBP-4)水平。结果:治疗后,中药辅助组临床总有效率(92.59%)高于西药组(77.78%)(P<0.05);中药辅助组BMI、WHR、FPG、FINS、TC、TG水平、血清LH、FSH、睾酮、雌二醇、瘦素、内脂素、IGFBP-3、RBP-4水平低于西药组(P<0.05)。结论:苍附导痰汤辅助二甲双胍、炔雌醇环丙孕酮片治疗青春期肥胖型PCOS患者可有效改善肥胖,调节糖脂代谢、性激素及调节病情相关因子水平,进而提升疗效,促进病情恢复。Objective:To investigate the clinical efficacy of Cangfu Daotan decoction assisted with metformin and ethinylestradiol cyproterone tablets in the treatment of adolescent obese polycystic ovary syndrome(PCOS).Methods:A total of 108 adolescent obese PCOS patients admitted to our hospital from Jan.2022 to Dec.2023 were collected and randomly divided into two groups,54 cases in each group.The western medicine group was treated with metformin and ethinylestradiol cyproterone tablets,and the traditional Chinese medicine(TCM)auxiliary group was treated with Cangfu Daotan decoction on this basis.After 3 months of treatment,the clinical efficacy,body mass index(BMI),waist hips ratio(WHR),fasting plasma glucose(FPG),fasting insulin(FINS),total cholesterol(TC),triglyceride(TG),serum levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol,testosterone,insulin growth factors binding protein-3(IGFBP-3),and retinol binding protein-4(RBP-4)were compared between the two groups.Results:After 3 months of treatment,the total effective rate of TCM auxiliary group(92.59%)was higher than that of the western medicine group(77.78%)(P<0.05);the levels of BMI,WHR,FPG,FINS,TC,TG,serum LH,FSH,testosterone,estradiol,leptin,lactone,IGFBP-3 and RBP-4 levels in TCM auxiliary group were lower than those in western medicine group(P<0.05).Conclusion:Cangfu Dao Tan decoction assisted with metformin and ethoestradiol cyproterone tablets in the treatment of adolescent obese PCOS patients can effectively improve obesity,regulate glucose and lipid metabolism,sex hormones and regulate the levels of disease related factors,and then improve the curative effect and promote the recovery of disease.
