基于围术期相关指标及并发症发生率探讨TRGPC与TGGPC治疗胆囊结石的临床价值

摘　　要：目的:探究经直肠入路保胆胆囊结石取出术(Trans-rectal gallbladder preserving cholecystolithotomy,TRGPC)与经胃入路保胆胆囊结石取出术(Trans-gastric gallbladder preserving cholecystolithotomy,TGGPC)治疗胆囊结石的应用价值。方法:回顾性收集2021年7月至2023年7月本院收治的82例胆囊结石患者的临床资料。根据手术方法的不同将患者分为对照组(经胃入路保胆胆囊结石取出术,n=39)和观察组(经直肠入路保胆胆囊结石取出术,n=43)。分析比较两组围术期相关指标、炎症反应[C反应蛋白(C-reactive protein,CRP)、白细胞计数(White blood cell count,WBC)、中性粒细胞百分比(Neutrophilic granulocyte percentage,N%)]及并发症发生率。结果:观察组的手术时间、术后流质饮食时间、住院时间均较对照组显著缩短(P<0.05)。术后3 d,观察组的CRP、WBC、N%均显著低于对照组(P<0.05)。观察组术后并发症总发生率显著低于对照组(P<0.05)。结论:相比TGGPC治疗方式,TRGPC能减轻炎症,促进术后恢复,减少并发症。Objective:To explore the value of Trans-rectal gallbladder preserving cholecystolithotomy,(TRGPC)and Trans-gastric gallbladder preserving cholecystolithotomy(TGGPC)in the treatment of gallbladder stones.Methods:The clinical data of 82 patients with cholecystolithiasis admitted to our hospital during 2021-07 to 2023-07 were retrospectively collected.According to the different surgical methods,the patients were divided into control group(gastric cholecystolithiasis preservation,n=39)and observation group(rectal cholecystolithiasis preservation,n=43).Perioperative relevant indexes,inflammatory response[C-reactive protein(CRP),White blood cell count(WBC),Neutrophilic granulocyte percentage(N%)]and complication rate were analyzed and compared between the two groups.Results:The operation time,postoperative liquid diet time and hospital stay time of observation group were significantly shortened compared with control group(P<0.05).After 3 days of operation,CRP,WBC and N%in observation group were significantly lower than those in control group(P<0.05).The total incidence of postoperative complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:Compared with TGGPC,TRGPC can reduce inflammation,promote postoperative recovery and reduce complications in the treatment of gallstones.
