县域医共体政策创新的可持续性研究

摘　　要：县域医共体是我国县域医疗资源配置制度的创新举措,如何促进县域医共体政策创新可持续发展是当前理论与实践较为关注的话题。研究运用系统理论思维,通过构建县域医共体创新系统框架,梳理了县域医共体的基本运行逻辑,并以Z县慢性病管理为例,讨论县域医共体可持续发展存在的困境,为该创新政策的可持续发展提出合理建议。研究结果表明,县域医共体实施中内外部环境所涉及的四个因素可能对其可持续发展产生消极或积极影响,包括配套制度建设、资源整合、各主体间利益关系以及公共卫生信息化建设水平,想要突破当前的政策实施困境,就需要从这四个方面着手努力。The county medical community represents an innovative approach to allocating medical resources at the county level in China,and promoting the sustainable development of this model is of great significance in both theory and practice.This paper applies systems theory to construct a framework for understanding and advancing the county medical community,analyzing its fundamental operational logic.Taking the example of chronic disease management in Z County,this paper explores the challenges involved in scaling up and sustaining this innovation,and offers recommendations for overcoming these obstacles.These findings highlight four critical factors within the internal and external environment that influence the sustainability of the county medical community,with both positive and negative impacts.These factors include the development of supportive systems,resource integration,the balance of interests among stakeholders,and the advancement of public health information infrastructure.To break through the current policy implementation dilemma,efforts must be made in these four areas.
