摘　　要：目的:探讨直肠高分辨MRI在评估直肠癌术前分期中的临床价值。方法:选取2022年1月-2024年1月经我院行直肠癌手术治疗的146例患者,回顾性收集其术前直肠高分辨MRI的T、N分期,与术后病理诊断比较。结果:采用Kappa法检验术前T分期准确率为89.73%,与病理诊断差异无统计学意义(P>0.05),Kappa值为0.80,一致性较好(P<0.01)。N分期的准确率为59.59%,与病理诊断差异有统计学意义(P<0.05),Kappa值为0.31(P<0.01),一致性较差。结论:高分辨率MRI是直肠癌术前分期的有力工具,尤其对T分期的诊断具有高准确率。尽管对N分期的诊断准确率较低,但该技术仍为直肠癌治疗提供了可靠的影像学依据。Objective:To investigate the clinical value of high-resolution rectal MRI in assessing preoperative staging of rectal cancer.Methods:A retrospective study was conducted on 146 patients who underwent rectal cancer surgery in our hospital between January 2022 and January 2024.We gathered high-resolution magnetic resonance imaging(MRI)data to determine the T and N staging of the patient's tumor and compared these findings with the postoperative pathological diagnosis.Results:High-resolution rectal MRI demonstrated an accuracy of 89.73%for preoperative T staging,which was not statistically significantly different from pathological diagnosis(P>0.05).A Kappa value of 0.80 indicated strong agreement(P<0.01).The accuracy of N staging was 59.59%,which was statistically significantly different from pathological diagnosis(P<0.05).A Kappa value of 0.31 indicated poor agreement(P<0.01).Conclusion:High-resolution rectal MRI provides accurate preoperative staging for rectal cancer patients,demonstrating high diagnostic value for T staging but relatively lower accuracy for N staging.High-resolution rectal MRI serves as a reliable imaging modality for rectal cancer treatment,offering significant clinical value.
