摘　　要：目的:探讨甲状腺球蛋白(Tg)及其抗体(TgAb)检测联合CT增强对甲状腺乳头状癌侧颈部淋巴结转移的预判价值。方法:回顾性分析2022年1月至2024年1月间58例甲状腺乳头状癌患者的临床资料,根据侧颈部淋巴结转移情况分组。采用ROC曲线分析甲状腺球蛋白、甲状腺球蛋白抗体及CT增强单独及联合预测的价值。结果:转移组与未转移组在肿瘤直径、Tg、TgAb水平上差异显著(P<0.05);CT增强预测灵敏度为76.19%,特异度为86.48%;ROC曲线分析显示,Tg、TgAb、CT增强单独预测AUC分别为0.842、0.856、0.813,联合预测AUC为0.946(P<0.05)。结论:Tg、TgAb检测联合CT增强对甲状腺乳头状癌侧颈部淋巴结转移具有良好的预判价值。Objective:To investigate the predictive value of thyroglobulin(Tg)and its antibody(TgAb)detection combined with CT enhancement in the prediction of cervical lymph node metastasis in papillary thyroid carcinoma.Methods:A retrospective analysis was conducted on the clinical data of 58 patients with papillary thyroid carcinoma admitted from January 2022 to January 2024.The patients were grouped based on the presence of lateral cervical lymph node metastasis.Receiver operating characteristic(ROC)curve analysis was performed to assess the predictive value of Tg,TgAb,CT enhancement alone,and their combination.Results:Significant differences were observed between the metastatic and non-metastatic groups in tumor diameter and levels of thyroglobulin and its antibody(P<0.05).CT enhancement demonstrated a sensitivity of 76.19%and a specificity of 86.48%in predicting lymph node metastasis.ROC curve analysis revealed area under the curve(AUC)values of 0.842,0.856,and 0.813 for Tg,TgAb and CT enhancement alone,respectively,while the AUC value for the combined prediction was 0.946(P<0.05).Conclusion:The combination of Tg,TgAb detection with CT enhancement demonstrates excellent predictive value for lateral cervical lymph node metastasis in papillary thyroid carcinoma,warranting its clinical application.
