摘 要:目的:评估便携式显示设备对判读乳腺X线摄影(mammography,MG)影像中肿块细节BI-RADS分类诊断准确性的影响。方法:利用乳腺仿真模体影像和50例经病理证实的MG乳腺肿块影像,通过云胶片软件在乳腺专用显示器、普通显示器、平板电脑(Pad)和智能手机上进行显示。将16名诊断医生分为4组,再随机分配4种显示设备进行独立判读,采用MedCalc20.0软件对数据进行统计学分析。结果:4种显示设备的主观性能评价在影像放大至适合观察的比例下阅读评分优于挂片协议软阅读的评分。乳腺专用显示器、普通显示器、Pad和智能手机的工作特征曲线下面积(AUC)值分别为0.875、0.857、0.886和0.861,表明Pad和智能手机在特定环境下可以满足BI-RADS分类诊断的需求。然而,智能手机的漏诊率较高,且Pad和智能手机对细线性或细树枝状钙化的检出率低于其他设备。结论:在特定观片环境下,智能手机和Pad基本可以满足对MG云胶片乳腺微小肿块BI-RADS分类诊断。推荐使用这些显示设备时,应将影像放大至适合的比例以优化诊断。Objective:To evaluate the impact of different display devices on the BI-RADS classification accuracy for breast micromasses in mammography(MG),so as to analyze the feasibility of the application of portable display device.Methods:We used the images of mammary gland monitor and 50 cases of MG breast mass confirmed by pathological diagnosis were selected from PACS network.These images were displayed on four different devices:a dedicated breast monitor,a common monitor,a tablet computer(Pad)and a smart phone.Sixteen radiologists were divided into 4 groups and randomly assigned 1 of 4 display devices for independent interpretation,and the data were statistically analyzed by MedCalc20.0 software.Results:The subjective performance of the four display devices indicated that reading scores were superior when images were magnified to an appropriate scale,compared to the hanging protocol soft reading method.The area under the receiver operating characteristic curve(AUC)values of dedicated breast monitor,common monitor,Pad and smart phone were 0.875,0.857,0.886 and 0.861,respectively,with no statistically significant differences(P>0.05).However,smart phone had a higher rate of missed diagnosis than other display devices,,and both the Pad and smartphone had lower detection rates for fine linear or fine dendritic calcifications.Conclusion:In a specific viewing environment,both the Pad and smart phone can basically meet the requirements for the BI-RADS classification accuracy for breast micromasses in mammography.It is recommended to magnify images to an appropriate scale when using these devices to optimize diagnostic accuracy.
