摘 要:目的:探究磁共振定量参数在2型糖尿病周围神经病变(DPN)中的应用价值。方法:选取2022年10月-2023年10月鄂尔多斯市中心医院收治的符合2型糖尿病相关诊断标准的30例糖尿病周围神经病变患者为DPN组、25例单纯糖尿病患者为糖尿病组、30例健康志愿者作为对照组,收集三组受试者的临床资料及糖尿病患者下肢电生理检查结果。所有受试者均行右侧膝关节水平扩散张量成像(DTI)及T2 mapping磁共振定量成像。通过图像后处理获取胫神经定量参数(FA、ADC、AD、RD、T2值)。采用单因素方差分析三组间计量资料差异性,并用LSD法进行事后分析,使用皮尔森系数评估各参数与MCV的相关性,若证实存在差异,则使用ROC曲线分析各定量参数对DPN的诊断效能。结果:DPN组胫神经FA值降低,ADC、RD和T2值升高,与对照组和糖尿病组有显著差异,AD值在三组间差异无统计学意义。FA与MCV正相关,ADC、RD和T2与MCV负相关。DTI和T2 mapping联合应用可进一步提高DPN诊断效能。结论:磁共振DTI和T2mapping成像可定量评估2型糖尿病周围神经病变的神经结构变化,二者联合应用进一步提高诊断效能,对DPN的临床诊断和治疗有重要意义。Objective:To investigate the application value of magnetic resonance quantitative parameters in diabetic peripheral neuropathy(DPN)in type 2 diabetes mellitus.Methods:Thirty patients diagnosed with DPN according to the diagnostic criteria for type 2 diabetes mellitus from October 2022 to October 2023 at the Erdos Central Hospital were selected as the DPN group.Twenty-five patients with uncomplicated type 2 diabetes mellitus were selected as the diabetes group,and 30 healthy volunteers were selected as the control group.Clinical data and results of lower limb electrophysiological examinations of the three groups were collected.All subjects underwent diffusion tensor imaging(DTI)and T2 mapping magnetic resonance quantitative imaging at the level of the right knee joint.Quantitative parameters of the tibial nerve(FA,ADC,AD,RD,T2 value)were obtained through image post-processing.One-way analysis of variance(ANOVA)was used to analyze the differences in metric data among the three groups,and post hoc analysis was performed using LSD method.Pearson correlation coefficient was used to evaluate the correlation between each parameter and MCV.If differences were confirmed,ROC curve analysis was used to evaluate the diagnostic efficacy of each quantitative parameter for DPN.Results:In the DPN group,the FA value of the tibial nerve decreased,while ADC,RD,and T2 values increased significantly compared to the control group and diabetes group.There was no statistically significant difference in AD values among the three groups.FA was positively correlated with MCV,while ADC,RD,and T2 were negatively correlated with MCV.The combined application of DTI and T2 mapping further improved the diagnostic efficacy for DPN.Conclusion:Magnetic resonance DTI and T2 mapping imaging can quantitatively evaluate the neurostructural changes in diabetic peripheral neuropathy in type 2 diabetes.Their combined application further enhances diagnostic efficacy,which is of great significance for the clinical diagnosis and treatment of DPN.
