基于病例组合指数的临床科室床位利用评价

摘　　要：目的应用DRG相关指标调整并评价医院床位效能,提高医院医疗质量和效益,促进床位资源合理利用。方法以新疆某三甲医院床位数据为基础,通过床位利用波士顿矩阵绘制散点图,根据计算床均RW值和测算平均住院日目标,计算CMI调整病床工作效率前后平均开放床位数的合理区间。结果利用波士顿矩阵绘制散点图发现,该院98个科室中,在第Ⅰ象限的“高效能”科室共有10个,在第Ⅱ象限为“高周转”科室的有34个,在第Ⅲ象限有19个科室为“低效能”科室,第Ⅳ象限为“高难度”科室有35个。结论DRG指标在床位效能评价中的应用,可以帮助医疗机构更好地管理床位资源、监控医疗质量、提高床位资源利用效率,为患者提供更加优质的医疗服务。Objective To apply DRG related indicators to adjust and evaluate the efficiency of hospital beds,improve the medical quality and efficiency of hospitals,and promote the rational use of bed resources.Methods Based on the data of beds in a tertiary hospital in Xinjiang,the Boston matrix was used to draw the scatter plot,and the reasonable interval of the average number of beds open before and after the adjustment of bed efficiency by CMI was calculated according to the average RW value of beds and the average length of stay.Results Using Boston matrix to draw scatter plot,it was found that among 98 departments in the hospital,there were 10"high efficiency"departments in the first quadrant,34"high turnover"departments in the second quadrant,19"low efficiency"departments in the third quadrant and 35"high difficulty"departments in the fourth quadrant.Conclusion The application of DRG index in bed efficiency evaluation can help medical institutions to better manage bed resources,monitor medical quality,improve the utilization efficiency of bed resources,and provide better medical services for patients.
