摘  要:目的:比较发散式体外冲击波(rESW)与针刺治疗肩周炎的疗效。方法:将肩周炎患者随机分为对照组、针刺组和ESW组,最终每组纳入20例,对照组患者行常规理疗,针刺组、ESW组患者分别在常规理疗基础上增加针刺、rESW治疗,2次/周,治疗8周。治疗前、治疗后4周、治疗后8周分别评估Constant-Murley肩关节评分(CMS)、美国肩肘外科评分(ASES)、SF-12简明生存质量量表评分。结果:与对照组相比,针刺组和ESW组在治疗4周和治疗8周时的CMS、ASES和SF-12评分提高,差异有统计学意义(均P<0.05);与针刺组相比,治疗8周时ESW组CMS和ASES评分更高。结论:经筋理论下rESW治疗肩周炎患者,可有效缓解肩疼痛、改善肩功能和日常生活活动能力。Objective:To compare the efficacy of radial extracorporeal shock wave(rESW)with acupuncture in the treatment of scapulohumeral periarthritis.Methods:Patients with scapulohumeral periarthritis were randomly divided into three groups:control,acupuncture,and rESW groups,with 20 cases in each ultimately.All groups received routine physiotherapy,while the acupuncture and rESW groups received additional treatments(2 sessions/week for 8 weeks).Outcomes were assessed using Chinese version of the Constant-Murley shoulder scoring scale(CMS),American shoulder and elbow surgeons scale(ASES),and 12-item short form health survey(SF-12)scores at baseline,4 weeks and 8 weeks after the therapy.Results:Compared to the control group,the acupuncture and rESW groups showed significant improvements in CMS,ASES,and SF-12 scores at both 4 and 8 weeks.At 8 weeks,the rESW group demonstrated higher CMS and ASES scores than the acupuncture group.Conclusion:rESW therapy based on meridian theory effectively relieves shoulder pain and improves function after 8 weeks.
