中性粒细胞淋巴细胞比值与成人心脏术后住院时间延长的相关性研究

摘　　要：目的探讨术前中性粒细胞淋巴细胞比值与成人心脏术后住院时间延长的相关性。方法回顾性分析2019年1-6月本院心外重症监护二科收治的成人心脏外科手术患者的临床资料。以中性粒细胞淋巴细胞比值对患者进行三等分组,分别为低值组(NLR<1.52)、中值组(1.52≤NLR≤2.19)和高值组(NLR>2.19)。通过单因素和多因素Logistic回归模型结合限制性立方样条模型,分析中性粒细胞淋巴细胞比值与成人心脏术后住院时间延长的关系。结果共纳入264例患者,多因素Logistic回归分析提示,以中性粒细胞淋巴细胞比值低值组为参照,术后住院时间延长的风险,中值组患者是2.56倍(OR=2.56,95%置信区间1.13~5.82,P=0.025);高值组患者是3.53倍(OR=3.53,95%置信区间1.60~7.82,P=0.002)。受试者工作特征曲线表明,术前中性粒细胞淋巴细胞比值预测术后住院时间延长的曲线下面积为0.635,95%置信区间为0.558~0.713,P<0.001。结论中性粒细胞淋巴细胞比值与成人心脏术后住院时间延长相关,是心脏术后住院时间延长的独立危险因素。Objective To explore the correlation between the preoperative neutrophil-to-lymphocyte ratio(NLR)and prolonged hospital stay in adult patients after cardiac surgery.Methods A retrospective analysis was conducted on the clinical data of adult patients who underwent cardiac surgery at the Second Department of Cardiac Surgical Intensive Care of Guangdong Provincial People's Hospital from January 2019 to June 2019.Patients were classified into three groups based on the NLR:low NLR(<1.52),middle NLR(1.52~2.19),and high NLR(>2.19).The relationship between NLR and prolonged hospital stay was analyzed using univariate and multivariate logistic regression models combined with restricted cubic spline models.Results A total of 264 patients were included in this study.Multivariate logistic regression analysis suggested that the risk of prolonged postoperative hospital stay was 2.56 times higher for patients with middle NLR levels(OR=2.56,95%CI 1.13~5.82,P=0.025)and 3.53 times higher for patients with high NLR levels(OR=3.53,95%CI 1.60~7.82,P=0.002),compared to the low NLR group.The receiver operating characteristic curve indicated that the area under the curve was 0.635 with a 95%CI of 0.558~0.713,P<0.001.Conclusions The neutrophil-to-lymphocyte ratio is an independent risk factor for prolonged hospital stay in adults after cardiac surgery and is associated with prolonged hospital stay.
