脑肠同调针刺法治疗急性缺血性脑卒中的疗效及对血清CGRP、5-HT、MTL水平的影响

摘　　要：目的研究脑肠同调针刺法治疗急性缺血性脑卒中(ACI)的疗效及对血清降钙素基因相关肽(CGRP)、胃动素(MTL)和5-羟色胺(5-HT)水平的影响。方法选择2023年1月-2024年1月本院收治的ACI患者74例作为研究对象,以随机信封法分为观察组和对照组两组,每组各37例。对照组行常规西医治疗,观察组行常规西医联合脑肠同调针刺治疗,两组均治疗1个月。比较两组患者治疗有效率;治疗前后中医症候积分、美国国立卫生院卒中量表(NIHSS)评分、运动功能(FMA)评分、血清指标(MTL、5-HT、CGRP)、炎症指标[白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)]及不良反应发生率。结果观察组治疗有效率较对照组高(P<0.05);治疗后中医症候积分、NIHSS评分、5-HT、IL-6、IL-1β水平较对照组低,FMA评分、CGRP、MTL水平较对照组高(均P<0.05);观察组不良反应率与对照组无明显差异(P>0.05)。结论使用脑肠同调针刺法对ACI患者进行治疗能够提高治疗有效率,促进患者运动功能恢复,减轻患者炎性反应及神经功能损伤。Objective To study the curative effect of brain-gut coherence acupuncture and its influences on serum calcitonin gene-related peptide(CGRP),motilin(MTL),and 5-hydroxytryptamine(5-HT)in acute cerebral infarction(ACI).Methods A total of 74 patients with ACI admitted to our hospital from January 2023 to January 2024 were enrolled and divided into a control group(37 cases,routine western medicine)and an observation group(37 cases,routine western medicine combined with brain-gut coherence acupuncture)using the random envelope method.All patients were treated for 1 month.The treatment response rate,scores of TCM symptoms,National Institutes of Health Stroke Scale(NIHSS),motor function(FMA),serum indexes(MTL,5-HT,CGRP),inflammatory indexes[interleukin-6(IL-6),interleukin-1β(IL-1β)],and incidence of adverse reactions were compared between the two groups before and after treatment.Results The response rate of treatment in observation group was higher than that in control group(P<0.05).After treatment,scores of TCM syndromes and NIHSS,levels of 5-HT,IL-6 and IL-1βin observation group were lower than those in control group,while FMA score,CGRP and MTL levels were higher than those in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Brain-gut coherence acupuncture can enhance the treatment response rate,promote the recovery of motor function,and alleviate inflammatory response and nerve function injury in ACI patients.
