摘 要:目的分析能谱CT单能量成像技术在职业性尘肺病诊断中的应用价值。方法选取2022年4月至2024年4月我院收治的180例疑似尘肺病患者作为研究对象,所有患者均给予常规CT与能谱CT检查,记录两种检查方式的成像特征并判断诊断价值。结果180例疑似尘肺病患者依据《尘肺病诊断标准》判定为职业性尘肺病140例,非尘肺病患者40例。能谱CT单能量图像对尘肺病患者特征性征象检出率均高于常规CT(P<0.05),其中40 keV单能量图像对肺部小结节和纤维化改变的检出率高于140 keV单能量图像(P<0.05);能谱CT单能量图像在显示肺部细微结构方面优于常规CT(P<0.05),其中40keV单能量图像在显示肺泡和支气管细微结构方面优于140 keV单能量图像(P<0.05);尘肺组在能量为40 keV、80 keV与140 keV的尘肺病灶的密度(平扫CT值)均高于非尘肺组(P<0.05);能谱CT 40 keV单能量图像对尘肺病的诊断准确率为95.00%(133/140),高于常规CT的87.14%(122/140),差异有统计学意义(χ^(2)=5.315,P=0.021)。结论能谱CT单能量成像技术在职业性尘肺病诊断中具有较高的临床应用价值,通过提高成像质量、特征性征象检出率和肺部细微结构显示能力,能谱CT单能量成像技术有助于医生更准确地判断职业性尘肺病患者的病情,制定更为合理的治疗方案。Objective To analyze the application value of energy spectrum CT single energy imaging technology in the diagnosis of occupational pneumoconiosis.Methods 180 patients with suspected pneumoconiosis admitted to our hospital from April 2022 to April 2024 were selected as the research objects.All patients were given routine CT and energy spectrum CT examination.The imaging characteristics of the two examination methods were recorded and the diagnostic value was judged.Results 140 cases of occupational pneumoconiosis and 40 cases of non-pneumoconiosis in 180 patients with suspected pneumoconiosis were diagnosed according to the diagnostic criteria of pneumoconiosis.The detection rate of characteristic signs of pneumoconiosis patients by energy spectrum CT monochromatic images was higher than that by conventional CT(P<0.05),and the detection rate of pulmonary nodules and fibrosis changes by 40 keV monochromatic images was higher than that by 140 keV monochromatic images(P<0.05).Energy spectrum CT monoenergetic images were superior to conventional CT in displaying the fine structure of the lung(P<0.05),and 40 keV monoenergetic images were superior to 140 keV monoenergetic images in displaying the fine structure of the alveoli and bronchi(P<0.05).The density(Plain CT value)of pneumoconiosis lesions with energy of 40 keV,80 keV and 140 keV in pneumoconiosis group was higher than that in non-pneumoconiosis group(P<0.05).The diagnostic accuracy of energy spectrum CT 40 keV monochromatic images for pneumoconiosis was 95.00%(133/140),higher than 87.14%(122/140)of conventional CT,with statistically significant difference(χ^(2)=5.315,P=0.021).Conclusions Energy spectrum CT single energy imaging technology has high clinical application value in the diagnosis of occupational pneumoconiosis.By improving the imaging quality,the detection rate of characteristic signs and the display ability of lung fine structure,energy spectrum CT single energy imaging technology can help doctors more accurately judge the condition of patients wit
