摘  要:目的:探讨血清长链非编码核糖核酸尿路上皮癌相关基因1(lncRNA-UCA1)、微小核糖核酸(miR)-143表达水平及与细胞凋亡、放疗敏感性的关系.方法:选择2021年6月至2023年12月我院142例宫颈癌患者(宫颈癌组)和142例正常宫颈组织患者(对照组).根据放疗疗效将宫颈癌组分为放疗敏感组(n=88)和放疗抵抗组(n=54),比较各组血清lncRNA-UCA1、miR-143、细胞凋亡指数.分析血清lncRNA-UCA1、miR-143与细胞凋亡的相关性.多因素Logistic回归分析宫颈癌患者放疗敏感性的影响因素,受试者工作特征(ROC)曲线评估血清lncRNA-UCA1、miR-143表达水平对宫颈癌患者放疗敏感性的预测价值.结果:宫颈癌组血清lncRNA-UCA1、细胞凋亡指数均高于对照组,miR-143低于对照组(P<0.05).血清lncRNA-UCA1与细胞凋亡指数呈正相关(r=0.531,P<0.05),血清miR-143与细胞凋亡指数呈负相关(r=-0.486,P<0.05).放疗敏感组和放疗抵抗组淋巴结转移、分化程度、国际妇产科联盟(FIGO)分期、血清lncRNA-UCA1、miR-143表达水平、细胞凋亡指数对比具有统计学差异(P<0.05).发生淋巴结转移、中/低分化、FIGO分期ⅢA~ⅢB期、细胞凋亡指数升高、血清lncRNA-UCA1表达水平升高是影响宫颈癌患者放疗敏感性的危险因素(P<0.05),miR-143表达水平升高则为保护因素(P<0.05).淋巴结转移、分化程度、FIGO分期、细胞凋亡指数、血清lncRNA-UCA1、miR-143单独及联合预测宫颈癌患者放疗敏感性的曲线下面积(AUC)分别为0.650、0.694、0.682、0.699、0.834、0.786、0.885,联合检测的预测效能更高.结论:宫颈癌患者血清lncRNA-UCA1表达水平升高、miR-143表达水平下降,与宫颈癌细胞凋亡和放疗敏感性降低有关,二指标联合检测对宫颈癌患者放疗敏感性具有较高的预测价值.Objective:To investigate the expression levels of serum long-chain noncoding ribonucleic acid urothelial carcinoma-associated gene 1(lncRNA-UCA1)and microribonucleic acid-143(miR)-143 and their relationship with apoptosis and radiotherapy sensitivity.Methods:142 cervical cancer patients(cervical cancer group)and 142 patients with normal cervical tissue(control group)in our hospital from June 2021 to December 2023 were selected.The cervical cancer group was divided into radiotherapy sensitive group(n=88)and radiotherapy resistant group(n=54)according to the radiotherapy effect,serum lncRNA-UCA1,miR-143 and the apoptosis index among different groups was compared.The correlation between serum lncRNA-UCA1,miR-143 and apoptosis were analyzed.The influencing factors of radiotherapy sensitivity in cervical cancer patients were analyzed by multivariate logistic regression analysis,the predictive value of the expression levels of serum lncRNA-UCA1 and miR-143 on radiotherapy sensitivity in cervical cancer patients was evaluated by receiver operating characteristic(ROC)curve.Results:The serum lncRNA UCA and apoptosis index in the cervical cancer group were higher than those in the control group,while miR-143 was lower than that in the control group(P<0.05).The serum lncRNA-UCA1 was positively correlated with the apoptosis index(r=0.531,P<0.05),while serum miR-143 was negatively correlated with the apoptosis index(r=-0.486,P<0.05).There were statistically significant differences in lymph node metastasis,degree of differentiation,and International Federation of Gynecology and Obstetrics(FIGO)staging,serum lncRNA-UCA1,miR-143 and apoptosis index between radiotherapy sensitive group and radiotherapy resistant group(P<0.05).Lymph node metastasis,medium/low differentiation,FIGO stageⅢA~ⅢB,elevated apoptosis index and elevated serum lncRNA-UCA1 expression level were risk factors affecting the radiosensitivity of cervical cancer patients(P<0.05),elevated miR-143 expression level was protective factor(P<0.05).The area under the
