摘  要:目的:观察低频脉冲电刺激联合凯格尔运动训练对产后尿潴留产妇尿流动力学和盆底肌力的影响.方法:118例产后尿潴留产妇分为对照组(59例,凯格尔运动训练)、观察组(59例,凯格尔运动训练联合低频脉冲电刺激).对比两组疗效、尿流动力学指标、首次排尿时间、首次排尿量、首次排尿后膀胱残余尿量、盆底肌力正常率.结果:与对照组相比,观察组临床总有效率升高(P<0.05).与对照组相比,观察组首次排尿时间缩短,首次排尿量增加,首次排尿后膀脱残余尿量减少(P<0.05).观察组治疗后最大尿流率(MFR)、逼尿肌压(Pdet)、膀胱顺应性、尿道闭合压高于对照组(P<0.05).观察组的盆底肌力正常率高于对照组(P<0.05).结论:凯格尔运动训练、低频脉冲电刺激联合治疗可显著提升产后尿潴留产妇临床疗效,促进临床症状恢复,有效改善产后尿潴留产妇尿流动力学和盆底肌力.Objective:To observe the effect of low frequency pulse electrical stimulation combined with Kegel exercise training on urodynamics and pelvic floor muscle strength in parturients with postpartum urinary retention.Methods:118 parturients with postpartum urinary retention were divided into observation group(59 cases,low frequency pulse electrical stimulation combined with Kegel exercise training)and control group(59 cases,Kegel exercise training)according to random number table method.The efficacy,urodynamic indexes,first urination time,first urination volume,bladder residual urine volume after first urination,and normal rate of pelvic floor muscle strength were compared between two groups.Results:Compared with control group,the total clinical effective rate in observation group increased(P<0.05).Compared with control group,the first urination time in observation group was shortened,the first urination volume was increased,and the residual urine volume of the bladder after the first urination was reduced(P<0.05).The maximum urinary flow rate(MFR),detrusor pressure(Pdet),bladder compliance and urethral closed pressure in observation group were higher than those in control group after treatment(P<0.05).The normal rate of pelvic floor muscle strength in observation group was higher than that in control group(P<0.05).Conclusion:The combined treatment of Kegel exercise training and low frequency pulse electrical stimulation can significantly improve the clinical efficacy of postpartum urinary retention,promote the recovery of clinical symptoms,and effectively improve the urodynamics and pelvic floor muscle strength in parturients with postpartum urinary retention.
