健脾益肾汤联合复方α-酮酸对慢性肾脏病的治疗效果及对微炎症状态、肾功能的影响分析

摘　　要：目的:分析健脾益肾汤联合复方α-酮酸对慢性肾脏病(chronic kidney disease,CKD)的治疗效果及对微炎症状态、肾功能的影响.方法:应用抽签法将我院2021年1月到2023年12月的102例CKD患者分为对照组(51例)与联合组(51例).对照组采取复方α-酮酸治疗,联合组采取健脾益肾汤联合复方α-酮酸治疗.治疗2个月后比较两组临床疗效,治疗前后中医证候积分,微炎症状态,肾功能及免疫功能变化.结果:联合组总有效率较中医组高(P<0.05);两组治疗后中医证候积分降低,联合组低于对照组(P<0.05);与治疗前相比,两组治疗后IL-6、IFN-γ、Hs-CRP、IL-7水平更低,联合组低于对照组(P<0.05);与治疗后相比,两组治疗后肾小球滤过率(eGFR)、血肌酐(Scr)更高,联合组高于对照组(P<0.05),尿素氮(BUM)更低,联合组低于对照组(P<0.05);与治疗前相比,两组治疗后CD4^(+)/CD8^(+)、树突状细胞(CD)数更高,联合组高于对照组(P<0.05).结论:健脾益肾汤联合复方α-酮酸对CKD治疗效果显著,可减轻患者中医症状,改善微炎症状态,提升肾功能,并进一步改善患者免疫功能水平.Objective:To analyze the therapeutic effect of Jianpi Yishen Tang combined with compound alpha ketoacid on chronic kidney disease(CKD)and its impact on micro inflammatory status and renal function.Methods:Using the lottery method,102 CKD patients in our hospital from January 2021 to December 2023 were divided into a matched group(51 cases)and a combination of traditional Chinese and matched group(51 cases).The matched group received treatment with compound alpha ketoacid,while the Joint group received treatment with Jianpi Yishen Tang combined with compound alpha ketoacid.Compare the clinical efficacy between the two groups after 2 months of treatment,including changes in traditional Chinese medicine syndrome scores,micro inflammatory status,renal function,and immune function before and Post-treatment.Results:The total effective rate of the Joint group was higher than that of matched group(P<0.05);The TCM syndrome score decreased Post-treatment in both groups,and the Joint group was lower than the matched group(P<0.05);Compared with Pretherapy,the levels of IL-6,IFN-γ,Hs CRP,and IL-7 were lower in both groups Post-treatment,and the Joint group was lower than the matched group(P<0.05);Compared with post-treatment,the glomerular filtration rate(eGFR)and serum creatinine(Scr)were higher in both groups Post-treatment,and the Joint group was higher than the matched group(P<0.05).The blood urea nitrogen(BUM)was lower,and the Joint group was lower than the matched group(P<0.05);Compared with Pretherapy,the CD4^(+)/CD8^(+)and dendritic cell(CD)counts were higher in both groups Post-treatment,and Joint group was higher than the matched group(P<0.05).Conclusion:The combination of Jianpi Yishen Tang and compound alpha ketoacid has a significant therapeutic effect on CKD,which can alleviate traditional Chinese medicine symptoms,improve micro inflammatory status,enhance renal function,and further improve the patient's immune function level.
