清热凉血化湿方联合红藤灌肠液对溃疡性直肠炎(湿热下注证)患者疗效、免疫功能和生活质量的影响

摘　　要：目的:评价清热凉血化湿方联合红藤灌肠液对溃疡性直肠炎(湿热下注证)患者疗效.方法:将我院2020.1-2023.12收治的82例湿热下注型溃疡性直肠炎患者,分为对照组(41例)和观察组(41例),分别予以清热凉血化湿方、清热凉血化湿方+红藤灌肠液治疗.比较两组的临床疗效、免疫功能及生活质量等.结果:(1)观察组治疗后腹痛、腹泻、里急后重及黏液脓血便评分明显低于对照组,IBDQ评分高于对照组(P<0.05);(2)两组治疗后血清Th17、Th17/Treg低于治疗前,Treg高于治疗前,观察组与对照组比较具有相同趋势(P<0.05);(3)观察组愈显率(82.93%vs.36.59%)、治疗总有效率(100.00%vs.80.49%)显著高于对照组(P<0.05);(4)观察组半年复发率显著低于对照组(5.88%vs.33.33%,P<0.05).结论:清热凉血化湿方联合红藤灌肠液对湿热下注型溃疡性直肠炎疗效肯定,可有效缓解患者的中医证候,调节免疫功能,提升生活质量,预防疾病复发.Objective:To evaluate the efficacy of patients with ulcerative proctitis(damp heat syndrome)treated with Qingre Liangxue Huashi Formula combined with Hongteng enema.Methods:A total of 82 patients admitted from January 2020-202 December 2023 in our hospital was divided into control group(41 patients)and observation group(41 patients).They were treated with Qingre Liangxue Huashi Formula,Qingre Liangxue Huashi Formula,and Hongteng Enema Liquid,respectively.Compare the clinical efficacy,immune function,and quality of life between the two groups.Results:(1)The observation group had lower scores for abdominal pain,diarrhea,severe diarrhea,and mucous purulent bloody stools after treatment compared to the control group,while the IBDQ score was higher than the control group(P<0.05);(2)In both groups,serum Th 17 and Th 17/Treg were lower than before treatment and Treg was higher than before treatment,showing the same trend between the observed group and the control group(P<0.05).After treatment,the serum Th17 and Th17/Treg levels in the observation group were lower than those in the control group,while Treg levels were significantly higher than those in the control group(P<0.05);(3)The significant improvement rate(82.93%vs.36.59%)and total effective rate(100.00%vs.80.49%)of the observation group were higher than those of the control group(P<0.05);(4)The six-month recurrence rate of the observation group was lower than that of the control group(5.88%vs.33.33%,P<0.05).Conclusion:The combination of Qingre Liangxue Huashi Formula and Hongteng Enema Liquid has a definite therapeutic effect on damp heat type ulcerative proctitis,which can effectively alleviate patients'traditional Chinese medicine symptoms,regulate immune function,improve quality of life,and prevent disease recurrence.
