摘 要:目的探讨胸部CT肺血管参数对慢性阻塞性肺疾病(COPD)急性加重期患者病情程度的评估价值。方法选取我院收治的102例COPD急性加重期患者,基于体征和血气分析将其分为轻度组、中度组及重度组。所有患者均于入院前后24 h内行胸部CT检查,分析胸部CT肺血管参数对COPD急性加重期患者病情程度的评估价值。结果经病情评估102例患者中轻度组30例,占29.4%;中度组48例,占47.1%;重度组24例,占23.5%。重度组肺小血管横截面积占肺总面积百分比(%CSA)低于轻、中度组,主肺动脉与升主动脉直径比值(PA/A)及阈值-950以下低密度衰减区占肺总容积百分比(LAA%-950)高于轻、中度组,差异有统计学意义(P<0.05)。绘制受试者工作特征(ROC)曲线,结果显示%CSA、PA/A、LAA%-950单独及联合评估COPD急性加重期患者重度病情的曲线下面积(AUC)均>0.7,均有一定评估价值,且联合评估价值高于%CSA、PA/A、LAA%-950单独评估,差异有统计学意义(P<0.05)。结论胸部CT肺血管参数(%CSA、PA/A、LAA%-950)对COPD急性加重期患者病情程度具有一定评估价值。Objective To explore the value of chest computed tomography(CT)pulmonary vascular parameters in evaluating the severity of acute exacerbation of chronic obstructive pulmonary disease(COPD).Method A prospective study was conducted on 102 patients with acute exacerbation of COPD admitted to hospitals.Based on patient signs and blood gas analysis,they were divided into mild,moderate,and severe groups.All patients underwent chest CT examination within 24 hours before and after admission,and the evaluation value of chest CT pulmonary vascular parameters on the severity of COPD in patients with acute exacerbation was analyzed.Results According to the evaluation of the condition,among the 102 patients,30 cases were mild,accounting for 29.4%.There were 48 cases of moderate patients,accounting for 47.1%.There were 24 severe patients,accounting for 23.5%.The percentage of pulmonary small vessel cross-sectional area to total lung area(%CSA)in the severe group was lower than that in the mild and moderate groups.The ratio of main pulmonary artery to ascending aorta diameter(PA/A)and the percentage of low-density attenuation area below the threshold of-950 to total lung volume(LAA%-950)were higher in the severe group than in the mild and moderate groups(P<0.05).The receiver's operating characteristic(ROC)curve was drawn,and the results showed that the area under the curve(AUC)of%CSA,PA/A,LAA%-950 alone and in combination for evaluating severe illness in patients with acute exacerbation of COPD was all greater than 0.7,indicating a certain evaluation value.The combined evaluation value was higher than that of%CSA,PA/A,LAA%-950 alone(P<0.05).Conclusion Chest CT pulmonary vascular parameters(%CSA,PA/A,LAA%-950)have certain evaluation value for the severity of COPD in patients with acute exacerbation.
