基于血清循环miR-210-3p及超声心动图指标对急性心力衰竭患者的预后价值分析

摘　　要：目的探讨急性心力衰竭患者血清miR-210-3p表达水平及超声心动图指标对其预后的临床价值。方法选取我院收治的93例急性心力衰竭患者临床资料进行分析,根据随访结果将患者分为发生不良心血管事件23例作为发生组与未发生组70例,分析患者入院就诊时miR-210-3p表达水平及超声心动图参数情况,并分析上述指标对患者预后的临床价值。结果发生心血管不良事件与未发生患者超声心动图检测参数LVDd、LVDs、LAD、舒张参数、瓣膜参数、肺动脉压参数、EMPS总评分差异有统计学意义(P<0.05)。发生不良心血管事件患者miR-210-3p相对表达量明显高于未发生患者,差异有统计学意义(P<0.05)。经ROC曲线分析显示miR-210-3p、超声心动图的预测AUC分别达0.881、0.989;诊断敏感度分别为87.0%、99.2%,特异度分别为81.4%、92.9%。结论miR-210-3p、超声心动图对急性心力衰竭患者预后均具有较高的临床价值。Objective To analyze the expression level of serum miR-210-3p in patients with acute heart failure and the clinical value of echocardiographic indexes on its prognosis and to provide relevant reference for the diagnosis and rehabilitation of patients with this disease.Methods The clinical data of 93 patients with acute heart failure admitted to our hospital were analyzed.According to the follow-up results,the patients were divided into two groups:the group with adverse cardiovascular events(23 cases)and the group without adverse cardiovascular events(70 cases).To analyze Then,we analyzed the expression level of miR-210-3p and echocardiographic indexes when patients were admitted to hospital,as well as analyzed the clinical value of the above indexes on the prognosis of patients.Results There were statistically significant differences(P<0.05)in the parameters of LVDd,LVDs,LAD,diastolic parameters,valve parameters,pulmonary artery pressure parameters,and total EMPS score detected by echocardiography between patients with and without cardiovascular adverse events.The relative expression level of miR-210-3p in patients with adverse cardiovascular events was significantly lower than that in patients without events(P<0.05).ROC curve analysis showed that the predicted AUC of miR-210-3p and echocardiography was 0.881 and 0.989 respectively.The diagnostic sensitivity was 87.0% and 99.2% and the specificity was 81.4%and 92.9%,respectively.Conclusion miR-210-3p and echocardiography have high clinical value in predicting the prognosis of patients with acute heart failure.
