MSCT对腹内疝分期和主要征象的初步探讨

摘　　要：目的探讨多层螺旋CT(MSCT)对腹内疝分期和主要征象的临床价值。方法选取手术病理证实的30例腹内疝患者,分析其临床和影像学资料,将腹内疝的MSCT分期结果与手术病理结果进行对照。结果与手术病理结果相比较,MSCT对腹内疝的3个分期准确率分别为66.7%、77.8%和88.9%,总准确率为83.3%。根据腹内疝的共同病理基础,筛选出腹内疝的6种主要MSCT征象,包括肠管异位征、囊袋征、闭袢征、靶征、漩涡征、血性渗液征,检出率分别为70.0%、33.3%、56.7%、53.3%、20.0%、43.3%。结论腹内疝的MSCT 3个分期有助于准确评估患者的病情,6种主要MSCT征象的依次观察构成腹内疝MSCT诊断的一般流程。Objective To investigate the clinical value of intra-abdominal hernia on staging and its main signs with multislice spiral computer tomography(MSCT).Methods 30 patients were confirmed by surgery with intra-abdominal hernia,their clinical and imaging datas were retrospectively analyzed,including its results with MSCT compared to the surgical and pathological findings.Results Compared with the surgical pathological results,the accuracy of MSCT in staging intra-abdominal hernia in three stages was 66.7%,77.8%,and 88.9%,respectively,with a total accuracy of 83.3%.Based on the common pathological basis of intra-abdominal hernias,six main MSCT signs of intra-abdominal hernias were selected,including intestinal ectopic sign,cystic sign,closed loop sign,target sign,vortex sign,and bloody exudate sign.The detection rates of them were respectively 70.0%,33.3%,56.7%,53.3%,20.0%,and 43.3%.Conclusion The three stages of MSCT for intra-abdominal hernia are helpful in accurately evaluating the patient's condition,and the sequential observation of six main MSCT signs constitutes the general procedure for MSCT diagnosis of intra-abdominal hernia.
