药物球囊联合支架植入治疗股浅动脉病变的临床分析

摘  要:目的探讨药物球囊(PEB)与普通球囊经皮腔内血管成形术(PTA)后植入自扩张金属支架(BMS)治疗股浅动脉(superficial femoral artery,SFA)狭窄性病变后再狭窄风险的差异。方法选取50例患者(56处病变)分为PEB+BMS组和PTA+BMS组。首要终点为12个月后再狭窄,次要终点是病变血运重建或截肢后恢复。分析比较长病变(>100mm)与短病变、真腔与内膜下入路的优劣。结果PEB+BMS组和PTA+BMS组的平均病变长度分别为(93.12±57.34)mm和(96.02±61.16)mm,差异无统计学意义(P=0.773)。与PTA+BMS组相比,PEB+BMS组在1年内发生血管再狭窄的百分比差异有统计学意义(P=0.004),两组中均无截肢者。两组的血管病变特征及技术途径未见明显统计学差异。结论与PTA+BMS治疗股浅动脉病变相比,PEB+BMS能显著降低再狭窄和靶病变血运重建的风险,且该效果不受病变长度和再通技术的影响,值得在临床实践中推广应用。Objective The purpose of this study is to compare the eluting balloon(paclitaxel-eluting balloon,PEB)versus the conventional percutaneous transluminal angioplasty(conventional percutaneous transluminal angioplasty,PTA)posterior placement into a self-expansion nickel-titanium bare metal stent(bare-metal stent,BMS)for the superficial femoral artery difference in restenosis risk after stenotic lesions.Methods A total of 50 patients(56 segment lesions)were divided into the PEB+BMS and PTA+BMS groups.The primary endpoint was restenosis after 12 months,and the secondary endpoint was revascularization of the lesion or recovery after amputation.The advantages and disadvantages of long lesions(>100 mm)and short lesions,true lumen and inferior intimal approach were analyzed.Results The mean lesion length in PEB+BMS group was(93.12±57.34)mm and(96.02±61.16)mm in PTA+BMS group,respectively(P=0.773).The PTA+BMS group was not statistically different(P=0.004),and no major amputation occurred in either group.No significant statistical differences were found in vascular lesion characteristics and technical approach in the two groups.Conclusions Preexisting angioplasty dilation of a paclitaxel-eluting balloon before stent implantation reduces the occurrence of restenosis and target lesion revascularization compared with PTA+BMS in the treatment of complex superficial femoral artery(SFA)lesions regardless of the lesion length and recanalization technique.
