外剥内扎术与吻合器痔上黏膜环切术治疗严重痔疮的临床效果分析

摘　　要：目的:探究外剥内扎术与吻合器痔上黏膜环切术(PPH)治疗严重痔疮的效果。方法:选取2021年1月—2023年9月无锡市第二老年病医院收治的严重痔疮患者120例,以随机数字表法分为对照组(实施外剥内扎术治疗)和观察组(应用PPH治疗),各60例。比较两组治疗效果、疼痛程度、手术指标及并发症发生情况。结果:观察组治疗总有效率高于对照组(P=0.015)。术前,两组视觉模拟评分法评分比较,无统计学差异(P>0.05);术后,两组视觉模拟评分法评分均降低,观察组低于对照组(P<0.05)。观察组伤口愈合时间、手术时间、住院时间均短于对照组,术中输血量少于对照组(P<0.001)。观察组并发症发生率低于对照组(P=0.015)。结论:严重痔疮患者采用PPH治疗的效果优于外剥内扎术疗法,可有效减轻患者疼痛,利于患者康复,安全性较高。Objective:To investigate the efficacy of external hemorrhoidectomy with internal ligation versus procedure for prolapse and hemorrhoids(PPH)in the treatment of severe hemorrhoids.Methods:A total of 120 patients with severe hemorrhoids admitted to Wuxi Second Geriatric Hospital from January 2021 to September 2023 were randomly divided into a control group(treated with external hemorrhoidectomy with internal ligation)and an observation group(treated with PPH),with 60 cases in each group.The treatment efficacy,pain level,surgical indicators,and incidence of complications were compared between the two groups.Results:The total effective rate in the observation group was higher than that in the control group(P=0.015).Preoperatively,there was no significant difference in the Visual Analog Scale scores between the two groups(P>0.05).Postoperatively,the Visual Analog Scale scores in both groups decreased,with the observation group scoring lower than the control group(P<0.05).The wound healing time,operation time,and hospital stay in the observation group were shorter than those in the control group,and the intraoperative blood transfusion volume was less than that in the control group(P<0.001).The incidence of complications in the observation group was lower than that in the control group(P=0.015).Conclusion:The efficacy of PPH in the treatment of severe hemorrhoids is superior to that of external hemorrhoidectomy with internal ligation,effectively reducing patient pain,promoting recovery,and demonstrating high safety.
