摘　　要：目的:分析不同手术时机股骨近端防旋髓内钉(PFNA)内固定治疗老年股骨转子间骨折的效果。方法:选取2021年12月—2023年12月于右江民族医学院附属河池医院行PFNA内固定治疗的90例老年股骨转子间骨折患者为研究对象,骨折至手术时间≤48 h的患者为观察组(45例),骨折至手术时间>48 h的患者为对照组(45例)。对比两组围术期指标、髋关节功能、日常生活活动能力、并发症发生率及临床疗效。结果:观察组手术用时、住院时间短于对照组,术中出血量、住院费用低于对照组(P<0.05);术后3个月,两组Harris评分、健康调查简表评分升高,观察组高于对照组(P<0.05);观察组并发症发生率低于对照组(P=0.007);观察组治疗总有效率高于对照组(P=0.003)。结论:骨折后48 h内应用PFNA内固定治疗老年股骨转子间骨折的效果确切,可有效改善患者髋关节功能,缩短手术时间、住院时间,降低住院费用、并发症发生率,提高日常生活活动能力。Objective:To analyze the efficacy of proximal femoral nail antirotation(PFNA)internal fixation in treating intertrochan⁃teric fractures in elderly patients at different surgical timings.Methods:A total of 90 elderly patients with intertrochanteric fractures treated with PFNA internal fixation at Hechi Hospital Affiliated to Youjiang Medical University for Nationalities from December 2021 to December 2023 were selected as study subjects.Patients with a fracture-to-surgery time of≤48 hours were assigned to the observation group(n=45),while those with a fracture-to-surgery time of>48 hours were assigned to the control group(n=45).Perioperative indicators,hip joint function,activities of daily living,complication rates,and clinical efficacy were compared between the two groups.Results:The observation group had shorter operation time and hospital stay,as well as lower intraop⁃erative blood loss and hospitalization costs compared to the control group(P<0.05).At 3 months postoperatively,both groups showed increased Harris scores and Short Form Health Survey scores,with the observation group scoring higher than the control group(P<0.05).The complication rate in the observation group was lower than that in the control group(P=0.007).The total effective rate of treatment in the observation group was higher than that in the control group(P=0.003).Conclusion:PFNA internal fixation within 48 hours after fracture in elderly patients with intertrochanteric fractures demonstrates definite efficacy,effectively improving hip joint function,shortening operation time and hospital stay,reducing hospitalization costs and complication rates,and enhancing activities of daily living.
