腹腔镜联合胆道镜手术与开腹手术治疗胆总管结石的效果分析

摘　　要：目的:分析腹腔镜联合胆道镜手术与开腹手术治疗胆总管结石的效果。方法:选取2020年5月—2023年5月在常州市武进人民医院接受治疗的60例胆总管结石患者作为研究对象,随机分为观察组和对照组,每组30例。观察组接受腹腔镜联合胆道镜手术治疗,对照组接受开腹手术治疗。比较两组围术期指标、C反应蛋白水平及治疗效果。结果:观察组术中出血量少于对照组,术后首次排气时间、住院时间短于对照组,差异有统计学意义(P<0.001);术后1、3、7 d,两组C反应蛋白水平高于术前,观察组低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率高于对照组,差异有统计学意义(P=0.020)。结论:与开腹手术相比,腹腔镜联合胆道镜手术治疗胆总管结石的效果更优,具有术中出血量少、术后炎性反应轻、术后恢复快的优势。Objective:To analyze the efficacy of laparoscopic combined with choledochoscopic surgery versus open surgery in the treatment of choledocholithiasis.Methods:Sixty patients with choledocholithiasis treated at Changzhou Wujin People's Hospital from May 2020 to May 2023 were selected as the study subjects and randomly divided into an observation group and a control group,with 30 patients in each group.The observation group underwent laparoscopic combined with choledochoscopic surgery,while the control group underwent open surgery.Perioperative indicators,C-reactive protein levels,and treatment efficacy were compared between the two groups.Results:The intraoperative blood loss in the observation group was less than that in the control group,and the first postoperative exhaust time and hospital stay in the observation group were shorter than those in the control group,with statistically significant differences(P<0.001).At 1,3,and 7 days postoperatively,the C-reactive protein levels in both groups were higher than those before surgery,but the levels in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The total effective rate in the observation group was significantly higher than that in the control group(P=0.020).Conclusion:Compared with open surgery,laparoscopic combined with choledochoscopic surgery is more effective in the treatment of choledocholithiasis,with advantages of less intraoperative blood loss,milder postoperative inflammatory response,and faster postoperative recovery.
