改良式经尿道前列腺等离子电切术与经尿道前列腺电切术治疗良性前列腺增生的效果对比

摘　　要：目的:对比改良式经尿道前列腺等离子电切术(PKRP)、经尿道前列腺电切术(TURP)治疗良性前列腺增生(BPH)的效果。方法:选取2023年1—12月田东县中医医院收治的86例BPH患者,根据随机数表法分为对照组、观察组,各43例。对照组采用TURP治疗,观察组采用改良式PKRP治疗。比较两组手术指标、尿流动力学指标、国际前列腺症状量表、前列腺增生患者生活质量量表、术后并发症发生率。结果:观察组切除组织重量高于对照组,手术时间、拔管时间短于对照组,术中出血量低于对照组(P<0.001);术后3个月,两组膀胱顺应性、最大尿流率升高,残余尿量降低,观察组优于对照组(P<0.05);术后3个月,两组国际前列腺症状量表评分降低,前列腺增生患者生活质量量表评分升高,观察组优于对照组(P<0.05);观察组术后并发症发生率低于对照组(P=0.024)。结论:与TURP相比,改良式PKRP的手术时间较短,术中出血量少,能改善BPH患者的尿流动力学指标,提高生活质量,减少术后并发症。Objective:To compare the efficacy of modified transurethral plasmakinetic resection of the prostate(PKRP)and transurethral resection of the prostate(TURP)in the treatment of benign prostatic hyperplasia(BPH).Methods:From January to December 2023,86 patients with BPH admitted to Tiandong Hospital of Traditional Chinese Medicine were divided into a control group(n=43)and an observation group(n=43)according to a random number table.The control group was treated with TURP,while the observation group was treated with modified PKRP.The surgical indicators,urodynamic parameters,International Prostate Symptom Score,Quality of Life Instrument for Chronic Diseases-Benign Prostatic Hyperplasia score,and the incidence of postoperative complications were compared between the two groups.Results:The weight of resected tissue in the observation group was higher than that in the control group,the operation time and extubation time were shorter than those in the control group,and the intraoperative blood loss was lower than that in the control group(P<0.001).At 3 months postoperatively,bladder compliance and maximum urinary flow rate increased,and residual urine volume decreased in both groups,with the observation group showing better results than the control group(P<0.05).At 3 months postoperatively,the International Prostate Symptom Score decreased,Quality of Life Instrument for Chronic Diseases-Benign Prostatic Hyperplasia score increased in both groups,with the observation group showing better results than the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P=0.024).Conclusion:Compared with TURP,modified PKRP has a shorter operation time,less intraoperative blood loss,and can improve urodynamic parameters,enhance quality of life,and reduce postoperative complications in patients with BPH.
