中药涂搽联合运动疗法在寒湿痹阻型肩袖损伤患者关节镜松解术后的应用效果

摘　　要：目的:探讨中药涂搽联合运动疗法在寒湿痹阻型肩袖损伤患者关节镜松解术后的应用效果。方法:纳入2023年1月—2024年6月眉山市中医医院收治的80例肩袖损伤患者,以随机数字表法分成对照组和观察组,各40例。两组均采用关节镜松解术治疗,对照组术后接受运动疗法,观察组术后在运动疗法基础上联合中药涂搽治疗。比较两组美国加州大学肩关节评分(UCLA)、视觉模拟评分法(VAS)评分、Constant-Murley肩关节评分量表(CMS)评分。结果:治疗前,两组UCLA评分对比,差异无统计学意义(P>0.05);治疗后,两组UCLA评分升高,观察组高于对照组(P<0.001)。治疗前,两组VAS评分对比,差异无统计学意义(P>0.05);治疗后,两组VAS评分降低,观察组低于对照组(P<0.001)。治疗前,两组CMS评分对比,差异无统计学意义(P>0.05);治疗后,两组CMS评分升高,观察组高于对照组(P<0.001)。结论:中药涂搽联合运动疗法在寒湿痹阻型肩袖损伤患者关节镜松解术后的应用效果较好,可提高患者的肩关节功能,减轻疼痛。Objective:To investigate the application effect of traditional Chinese medicine(TCM)topical application combined with exercise therapy in patients with cold-dampness obstruction type rotator cuff injury after arthroscopic release.Methods:Eighty patients with rotator cuff injury admitted to Meishan Hospital of Traditional Chinese Medicine from January 2023 to June 2024 were randomly divided into a control group and an observation group,with 40 cases in each group.Both groups underwent arthroscopic release.The control group received postoperative exercise therapy,while the observation group received postoperative exercise therapy combined with TCM topical application.The University of California,Los Angeles(UCLA)Shoulder Score,Visual Analog Scale(VAS)score,and Constant-Murley Shoulder Score(CMS)were compared between the two groups.Results:Before treatment,there was no significant difference in UCLA scores between the two groups(P>0.05).After treatment,the UCLA scores of both groups increased,with the observation group scoring higher than the control group(P<0.001).Before treatment,there was no significant difference in VAS scores between the two groups(P>0.05).After treatment,the VAS scores of both groups decreased,with the observation group scoring lower than the control group(P<0.001).Before treatment,there was no significant difference in CMS scores between the two groups(P>0.05).After treatment,the CMS scores of both groups increased,with the observation group scoring higher than the control group(P<0.001).Conclusion:The combination of TCM topical application and exercise therapy demonstrates a better application effect in patients with cold-dampness obstruction type rotator cuff injury after arthroscopic release,improving shoulder joint function and reducing pain.
