摘　　要：目的:探讨桃核承气汤加味治疗老年髋部骨折后便秘的效果。方法:选取2023年4月—2024年3月北京王府中西医结合医院收治的髋部骨折后便秘老年患者,根据随机数字表法分为对照组(n=30,开塞露治疗)和观察组(n=30,开塞露联合桃核承气汤加味治疗)。对比两组症状评分、胃肠功能指标、临床指标、治疗效果。结果:治疗后,两组欲便不得、大便秘结、坠胀不尽、腹胀腹痛、四肢不温、小便短赤评分降低,观察组低于对照组(P<0.05);治疗后,两组胃泌素、胃动素、生长抑素水平升高,观察组高于对照组(P<0.05);观察组腹胀改善时间、首次排便时间、肛门排气时间早于对照组,排便频率、肠鸣音次数高于对照组(P<0.001);观察组治疗总有效率较对照组高(P=0.028)。结论:桃核承气汤治疗老年髋部骨折后便秘的效果较为显著,可促进患者症状消退与术后恢复,改善胃肠功能。Objective:To investigate the effect of modified Taohe Chengqi decoction on constipation after hip fracture in elderly patients.Methods:Elderly patients with constipation after hip fracture admitted to Beijing Wangfu Hospital of Integrated Traditional Chinese and Western Medicine from April 2023 to March 2024 were selected and randomly divided into a control group(n=30,treated with glycerin enema)and an observation group(n=30,treated with glycerin enema combined with modified Taohe Chengqi decoction).Symptom scores,gastrointestinal function indicators,clinical indicators,and treatment efficacy were compared between the two groups.Results:After treatment,scores for difficulty in defecation,constipation,incomplete evacuation,abdominal distension and pain,cold limbs,and scanty dark urine decreased in both groups,with the observation group showing lower scores than the control group(P<0.05).After treatment,levels of gastrin,motilin,and somatostatin increased in both groups,with the observation group showing higher levels than the control group(P<0.05).The improvement time of abdominal distension,first defecation time,and anal exhaust time in the observation group were earlier than those in the control group,while defecation frequency and bowel sound frequency were higher in the observation group than in the control group(P<0.001).The total effective rate in the observation group was higher than that in the control group(P=0.028).Conclusion:Modified Taohe Chengqi decoction is effective in treating constipation after hip fracture in elderly patients,promoting symptom relief and postoperative recovery,and improving gastrointestinal function.
