白细胞计数联合中性粒细胞计数对腹部手术后急性疼痛的诊断意义分析

摘　　要：目的:分析白细胞计数联合中性粒细胞计数对腹部手术后急性疼痛的诊断价值。方法:回顾性分析2021年1月—2022年12月会宁县人民医院80例腹部手术后疑似急性疼痛患者的临床资料,以临床综合诊断结果为“金标准”,分析白细胞计数与中性粒细胞计数单项检测以及两者联合检测与临床综合诊断结果的一致性,对腹部手术后急性疼痛的诊断价值。结果:中性粒细胞计数与临床综合诊断结果一致性尚可(Kappa=0.507,P=0.049),白细胞计数与临床综合诊断结果一致性尚可(Kappa=0.645,P=0.015),白细胞计数联合中性粒细胞计数与临床综合诊断结果一致性较好(Kappa=0.894,P=0.002)。联合检测灵敏度高于白细胞计数、中性粒细胞计数单项检测(P<0.05)。白细胞计数与中性粒细胞计数单项检测以及两者联合检测的特异度、阳性预测值、阴性预测值及准确率比较,差异无统计学意义(P>0.05)。结论:白细胞计数联合中性粒细胞计数检测诊断腹部手术后急性疼痛的一致性较高,能够提高诊断灵敏度。Objective:To analyze the diagnostic value of white blood cell count combined with neutrophil count for acute pain after abdominal surgery.Methods:The clinical data of 80 patients with suspected acute pain after abdominal surgery from January 2021 to December 2022 at Baiyin Huining People's Hospital were retrospectively analyzed.Using the comprehensive clinical diagnosis as the"gold standard",the consistency of single detection of white blood cell count and neutrophil count,as well as their combined detection with the comprehensive clinical diagnosis,their diagnostic value for acute pain after abdominal surgery.Results:The consistency between neutrophil count and clinical comprehensive diagnosis was acceptable(Kappa=0.507,P=0.049),and the consistency between white blood cell count and clinical comprehensive diagnosis was also acceptable(Kappa=0.645,P=0.015).The combined detection of white blood cell count and neutrophil count showed good agreement with the clinical comprehensive diagnosis(Kappa=0.894,P=0.002).The sensitivity of the combined detection was higher than that of single detection of white blood cell count and neutrophil count(P<0.05).There were no statistically significant differences in specificity,positive predictive value,negative predictive value,and accuracy between single detection and combined detection of white blood cell count and neutrophil count(P>0.05).Conclusion:The combined detection of white blood cell count and neutrophil count has high consistency in diagnosing acute pain after abdominal surgery and can improve diagnostic sensitivity.
