自拟疏运化滞汤治疗脾虚气滞证功能性消化不良餐后不适综合征的临床效果

摘　　要：目的:探讨自拟疏运化滞汤治疗脾虚气滞证功能性消化不良餐后不适综合征(postprandial distress syndrome,PDS)患者的临床效果。方法:选取2020年1月—2024年6月厦门大学附属中山医院收治的88例脾虚气滞证功能性消化不良PDS患者,随机分组法均分为两组,其中给予常规西药治疗的为对照组,给予自拟疏运化滞汤治疗的研究组。比较两组疗效、中医症候积分、生活质量、心理状态[汉密尔顿抑郁量表(Hamilton depression scale,HAMD)评分、汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)评分]及复发情况。结果:研究组总有效率90.91%,高于对照组的72.73%,差异有统计学意义(P<0.05)。研究组治疗后各症状评分及中医症候总积分低于对照组,差异有统计学意义(P<0.05)。两组治疗后生活质量各维度评分及总分高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05)。两组治疗后HAMD评分、HAMA评分低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05)。研究组复发率5.00%,低于对照组的21.88%,差异有统计学意义(P<0.05)。结论:自拟疏运化滞汤能够对脾虚气滞证功能性消化不良PDS患者临床症状和生活进行改善,减少复发,提升疗效。Objective:To explore clinical effect of self-prepared Shuyun Huazhi Decoction in the treatment of postprandial distress syndrome(PDS)of functional dyspepsia with spleen deficiency Qi stagnation syndrome.Method:A total of 88 PDS patients with functional dyspepsia due to spleen deficiency and Qi stagnation syndrome treated in Zhongshan Hospital Affiliated to Xiamen University from January 2020 to June 2024 were selected and divided into two groups by random grouping method.The control group was given conventional western medicine treatment and the study group was given self-prepared Shuyun Huazhi Decoction treatment.The efficacy,traditional Chinese medicine syndrome score,quality of life,mental state[Hamilton depression scale(HAMD)score,Hamilton anxiety scale(HAMA)score]and recurrence were compared between the two groups.Result:The total effective rate of the study group was 90.91%,which was higher than 72.73%of the control group,the difference was statistically significant(P<0.05).After treatment,the scores of symptoms and total scores of traditional Chinese medicine symptoms in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The scores and total scores of all dimensions of quality of life in the two groups after treatment were higher than before treatment,and the study group was higher than the control group,the differences were statistically significant(P<0.05).HAMD scores and HAMA scores of the two groups after treatment were lower than those before treatment,and the study group was lower than the control group,the differences were statistically significant(P<0.05).The recurrence rate in the study group was 5.00%,lower than 21.88%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Self-made Shuyun Huazhi Decoction can improve the clinical symptoms and life of PDS patients with functional dyspepsia of spleen deficiency Qi stagnation syndrome,reduce recurrence and enhance curative effect.
