摘 要:目的:分析隐匿性骨折患者采取1.5T超导磁共振(MRI)与64排螺旋CT诊断的临床应用价值。方法:选取2019年1月—2024年1月荆门市中医医院拟诊为隐匿性骨折的86例患者作为研究对象,所有患者均接受1.5T超导MRI与64排螺旋CT检查,以手术或关节镜结果为标准,比较两种检查方式诊断结果、不同部位隐匿性骨折检出情况、诊断效能及主要影像学指标检出情况。结果:手术或关节镜检查确诊隐匿性骨折患者78例,64排螺旋CT检查误诊2例,漏诊8例,1.5T超导MRI误诊1例,漏诊2例。手术或关节镜检查出四肢关节骨折48例,肋骨骨折22例,髋关节骨折8例,1.5T超导MRI不同部位隐匿性骨折检出率高于64排螺旋CT,差异有统计学意义(P<0.05)。1.5T超导MRI诊断敏感度、准确度高于64排螺旋CT,差异有统计学意义(P<0.05);两种检查方式特异度、阳性预测值、阴性预测值比较,差异无统计学意义(P>0.05)。1.5T超导MRI软组织损伤、骨髓水肿、骨质损伤征象检出率高于64排螺旋CT,差异有统计学意义(P<0.05)。结论:隐匿性骨折患者采取1.5T超导MRI与64排螺旋CT诊断均具有较高的临床应用价值,MRI诊断不同部位隐匿性骨折及主要影像学指标检出率较高,同时具有更高的诊断效能,能有效提升诊断准确度。Objective:To analyze the clinical value of 1.5T superconducting magnetic resonance(MRI)and 64-slice spiral CT in diagnosing patients with occult fractures.Method:A total of 86 patients with occult fracture admitted in Jingmen Hospital of Traditional Chinese Medicine from January 2019 to January 2024 were included in the study,all of whom were diagnosed with 1.5T superconducting magnetic resonance and 64-slice spiral CT.The results of surgery or arthroscopy were used as the standard to compare the diagnostic results of the two methods,the detection of occult fractures in different parts,the diagnostic efficiency and the detection of main imaging indicators.Result:There were 78 patients with occult fracture confirmed by surgery or arthroscopy,2 patients were misdiagnosed by 64-slice spiral CT and 8 were missed,1 patient was misdiagnosed by 1.5-T superconducting MRI and 2 were missed.Operative or arthroscopic examination revealed 48 cases of limb joint fractures,22 cases of rib fractures,and 8 cases of hip fractures.The detection rate of occult fractures in different parts of 1.5T superconducting MRI was higher than that of 64-row spiral CT,the differences were statistically significant(P<0.05).The diagnostic sensitivity and accuracy of 1.5T superconducting MRI were higher than those of 64-row spiral CT,and the differences were statistically significant(P<0.05).There was no significant difference in the specificity,positive predictive value and negative predictive value between the two methods(P>0.05).The detection rate of soft tissue injury,bone marrow edema and bone injury in 1.5T superconducting MRI was higher than that in 64-row spiral CT,and the difference was statistically significant(P<0.05).Conclusion:Both 1.5T superconducting MRI and 64-slice spiral CT have high clinical application value in the diagnosis of patients with occult fractures.MRI diagnosis has a higher detection rate for different sites of occult fractures and main imaging indicators,and has higher diagnostic efficiency,which can effectively im
