摘　　要：痛经是育龄期女性最常见疾病之一,根据有无盆腔器质性疾病将其分为原发性痛经和继发性痛经,其中原发性痛经病因不明确,且不伴盆腔器质性疾病。张伶俐教授临证30余载,在治疗原发性痛经方面具有丰富的经验和独特的见解,谨守“不通则痛”和“不荣则痛”的基本病机,注重审证求因,分型论治。实证为肝郁气滞,气滞则生血瘀,痰湿,不通则痛;虚证则为肝血亏虚,不荣则痛;遵循“急则治标,缓则治本”的原则,痛经发作时以止痛为主,痛经缓解时以固本为主。张伶俐教授认为妇女疾病的发生与平素肝郁常多较为密切,所以在治法上须条达为要,并且组方灵活,善用疏肝解郁之药物,以调理肝脏气机为中心,根据不同的病因及所涉及到的其他脏腑,侧重不同的治法,在临床中取得了较好的疗效。Dysmenorrhea is one of the most common diseases in women of childbearing age,according to the presence or absence of pelvic organic diseases,it is divided into primary dysmenorrhea and secondary dysmenorrhea.The etiology of primary dysmenorrhea is not clear,and it is not accompanied by pelvic organic diseases.Professor Zhang Lingli has been in clinical practice for more than 30 years,she has rich experience and unique insights in the treatment of primary dysmenorrhea.She adheres to the basic pathogenesis of"pain due to obstruction"and"pain due to lack of glory",and pays attention to examining symptoms to determine the cause and treating based on syndrome differentiation.The excess syndrome is liver depression and Qi stagnation,Qi stagnation leads to blood stasis,phlegm dampness,obstruction leads to pain;deficiency syndrome is liver blood deficiency,lack of glory leads to pain;according to the principle of"treating the symptoms when it is urgent,and treating the root when it is slow",pain relief is the main method when dysmenorrhea occurs,and strengthening the root is the main method when dysmenorrhea is relieved.Professor Zhang Lingli believes that the occurrence of women's disease is often closely related to the liver depression,so it is necessary to be unobstructed in the treatment,and to be flexible in prescription.She is good at using the drugs of soothing liver and relieving depression,focusing on regulating liver Qi movement mechanism.According to different etiology and other viscera organs involved,she focuses on different treatment methods,which has achieved good results in clinic.
