摘 要:目的研究MRI联合超声在宫颈病变性质判断及病情严重程度评估中的应用价值,以期为宫颈病变的诊断提供有效手段。方法选取2021年3月至2023年3月医院收治的80例宫颈病变患者作为研究对象。所有患者均进行MRI检查、超声检查及病理检查。将病理检查结果作为金标准,比较MRI、超声单独及二者联合检查对恶性宫颈病变的诊断效能;比较MRI、超声单独及二者联合检查对恶性宫颈病变临床分期及宫旁侵犯的检出情况。结果病理检查显示,恶性宫颈病变42例,良性宫颈病变38例。MRI共检出恶性宫颈病变42例,其中7例漏诊,7例误诊;超声共检出恶性宫颈病变41例,其中9例漏诊,8例误诊;MRI联合超声共检出恶性宫颈病变42例,其中1例漏诊,1例误诊。MRI联合超声诊断恶性宫颈病变的灵敏度、特异度、准确度分别为97.62%、97.37%、97.50%,高于MRI的83.33%、81.58%、82.50%以及超声的78.57%、78.95%、78.75%(P<0.05)。MRI联合超声对恶性宫颈病变患者临床分期的检出总符合率为92.86%,高于MRI的71.43%及超声的61.90%(P<0.05)。MRI联合超声及MRI对恶性宫颈病变患者宫旁侵犯的检出总符合率均高于超声检查,差异具有统计学意义(P<0.05)。结论MRI联合超声可有效判断宫颈病变性质,且能提高恶性宫颈病变临床分期及宫旁侵犯评估的准确性。Objective To study the application value of MRI combined with ultrasound in the judgment of the nature of cervical lesions and the evaluation of the severity of the disease,in order to provide an effective means for the diagnosis of cervical lesions.Methods A total of 80 patients with cervical lesions admitted in the hospital from March 2021 to March 2023 were selected as the research objects.All patients underwent MRI examination,ultrasound examination and pathological examination.The results of pathological examination was regarded as the gold standard,and the diagnostic efficacy of MRI,ultrasound single and combined examination in malignant cervical lesions was compared;the detection of clinical staging and parametrial invasion of malignant cervical lesions by MRI,ultrasound single and combined examination were compared.Results Pathological examination showed 42 cases of malignant cervical lesions and 38 cases of benign cervical lesions;a total of 42 cases of malignant cervical lesions were detected by MRI,of which 7 cases were missed and 7 cases were misdiagnosed;a total of 41 cases of malignant cervical lesions were detected by ultrasound,of which 9 cases were missed and 8 cases were misdiagnosed;a total of 42 cases of malignant cervical lesions were detected by MRI combined with ultrasound,of which 1 case was missed and 1 case was misdiagnosed.The sensitivity,specificity and accuracy of MRI combined with ultrasound in the diagnosis of malignant cervical lesions were 97.62%,97.37% and 97.50%,respectively,which were higher than 83.33%,81.58% and 82.50% of MRI and 78.57%,78.95% and 78.75% of ultrasound(P<0.05).The total coincidence rate of MRI combined with ultrasound in the detection of clinical staging of patients with malignant cervical lesions was 92.86%,which was higher than 71.43% of MRI and 61.90% of ultrasound(P<0.05).The total coincidence rate of MRI combined with ultrasound and MRI in the detection of parametrial invasion in patients with malignant cervical lesions was higher than that of ultrasound,
