黄连解毒汤联合甘草酸二铵治疗非酒精性脂肪性肝炎(湿热蕴结证)的效果及对脂代谢、肝功能的影响

摘　　要：目的探讨黄连解毒汤联合甘草酸二铵治疗非酒精性脂肪性肝炎(NASH)(湿热蕴结证)的效果及对脂代谢、肝功能的影响。方法选取2021年4月至2023年4月收治的100例NASH(湿热蕴结证)患者为研究对象,随机将其分为对照组和观察组,每组50例。对照组给予甘草酸二铵治疗,观察组在对照组基础上加用黄连解毒汤治疗。比较两组的中医证候积分、脂代谢指标及肝功能指标。结果治疗后,观察组的肋痛、脘腹胀满、口黏腻评分及总分低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平低于对照组,高密度脂蛋白胆固醇(HDL-C)水平高于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)、碱性磷酸酶(ALP)水平低于对照组,差异具有统计学意义(P<0.05)。结论黄连解毒汤联合甘草酸二铵治疗NASH(湿热蕴结证)的效果显著,不仅能调节脂代谢,还能促进肝功能恢复,值得推广。Objective To explore the effect of Huanglian Jiedu decoction combined with diammonium glycyrrhizinate in the treatment of nonalcoholic steatohepatitis(NASH)(damp-heat accumulation syndrome)and its influences on lipid metabolism and liver function.Methods A total of 100 patients with NASH(damp-heat accumulation syndrome)admitted from April 2021 to April 2023 were selected as the study objects and randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with diammonium glycyrrhizinate,and the observation group was treated with Huanglian Jiedu decoction on the basis of the control group.The TCM syndrome scores,lipid metabolism indexes and liver function indexes of the two groups were compared.Results After treatment,the scores of rib pain,abdominal distension,sticky mouth and total score in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of triglyceride(TG),total cholesterol(TC)and low density lipoprotein-cholesterol(LDL-C)in the observation group were lower than those in the control group,the level of high density lipoprotein-cholesterol(HDL-C)was higher than that in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of aspartate aminotransferase(AST),alanine aminotransferase(ALT),total bilirubin(TBIL)and alkaline phosphatase(ALP)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Huanglian Jiedu decoction combined with diammonium glycyrrhizinate has a significant effect in the treatment of NASH(damp-heat accumulation syndrome),it can not only regulate lipid metabolism,but also promote the recovery of liver function,which is worthy of promotion.
