摘　　要：目的探讨音乐分娩疗法结合导乐陪伴及自由体位分娩对初产妇分娩恐惧及分娩信心的影响。方法选择2022年2月至2023年2月的98例初产妇作为研究对象,按照随机数字表法将其分为对照组和观察组,每组49例。对照组接受常规助产护理,观察组在对照组基础上实施音乐分娩疗法结合导乐陪伴及自由体位分娩。比较两组的干预效果。结果干预后,观察组的分娩恐惧量表(CAQ)、恐惧视觉模拟量表(FAVS)评分低于对照组,差异具有统计学意义(P<0.05)。干预后,观察组的自我效能感量表(GSES)、分娩信心评分高于对照组,差异具有统计学意义(P<0.05)。观察组的第一产程、第二产程短于对照组,差异具有统计学意义(P<0.05);两组的第三产程比较,差异无统计学意义(P>0.05)。观察组的不良妊娠结局总发生率低于对照组,差异具有统计学意义(P<0.05)。结论音乐分娩疗法结合导乐陪伴及自由体位分娩可有效减轻初产妇的分娩恐惧感,提高分娩信心,进而缩短产程,改善分娩结局,值得推广。Objective To explore the effects of music delivery therapy combined with doula accompany and free position delivery on childbirth fear and childbirth confidence of primipara.Methods A total of 98 primiparas from February 2022 to February 2023 were selected as the research objects and divided into control group and observation group according to the random number table method,with 49 cases in each group.The control group was given routine midwifery nursing,while the observation group was given music delivery therapy combined with doula accompany and free position delivery on the basis of the control group.The intervention effects of the two groups were compared.Results After intervention,the scores of Childbirth Attitudes Questionnaires(CAQ)and Fear Visual Analogue Scale(FAVS)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the General Self-Efficacy Scale(GSES)and childbirth confidence scores of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The first stage of labor and the second stage of labor in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05);there was no significant difference in the third stage of labor between the two groups(P>0.05).The total incidence of adverse pregnancy outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Music delivery therapy combined with doula accompany and free position delivery can effectively reduce the fear of childbirth of primipara,improve the confidence of childbirth,shorten the labor process and improve the outcome of childbirth,which is worthy of promotion.
