血清sE-selectin、IL-36γ、IL-31水平与寻常型银屑病的相关性

摘　　要：目的探讨寻常型银屑病(PsV)患者血清可溶性E选择素(sE-selectin)、白介素-36γ(IL-36γ)、白介素-31(IL-31)水平对疾病转归的评估价值。方法选取103例PsV患者(设为观察组)进行前瞻性研究,另选取同期健康志愿者103例设为对照组。比较两组血清sE-selectin、IL-36γ、IL-31水平,分析其与临床指标、病情程度相关性。PsV患者予以常规治疗,依据治疗4周后疾病转归分为转归不良亚组、转归良好亚组,比较其血清sE-selectin、IL-36γ、IL-31水平,分析其对疾病转归的评估价值。结果观察组血清sE-selectin、IL-36γ、IL-31水平高于对照组,且中重度者高于轻度者,差异有统计学意义(P<0.05);血清sE-selectin、IL-36γ、IL-31与中性粒细胞数计数、白细胞计数、病情程度、ESR、hs-CRP水平呈正相关(P<0.05);转归不良亚组血清sE-selectin、IL-36γ、IL-31水平高于转归良好亚组(P<0.05);血清sE-selectin、IL-36γ、IL-31水平联合评估疾病转归的AUC值大于单项指标评估(P<0.05)。结论PsV患者血清sE-selectin、IL-36γ、IL-31水平升高,且与病情程度呈正相关,联合检测其水平对疾病转归具有一定评估价值。Objective To investigate the value of soluble E-selectin(sE-selectin),interleukin-36γ(IL-36γ),and interleukin-31(IL-31)levels in the serum of patients with psoriasis vulgaris(PsV)in evaluating disease outcome.Methods 103 patients with PsV(set as the observation group)were selected for the prospective study,and 103 healthy volunteers in the same period were selected as the control group.The serum levels of sE-selectin,IL-36γand IL-31 were compared between the two groups and analyzed their correlation with clinical indicators and the degree of disease.Patients with PsV were treated as usual,and were divided into poor outcome subgroup and good outcome subgroup after 4 weeks of treatment.The serum levels of sE-selectin,IL-36γand IL-31 were compared,and the evaluation value of the disease outcome was analyzed.Results The levels of serum sE-selectin,IL-36γ,and IL-31 in the observation group were higher than those in the control group,and the levels in moderate and severe cases were higher than those in mild cases,showing a statistically significant difference(P<0.05).Serum sE-selectin,IL-36γ,and IL-31 were positively correlated with neutrophil count,white blood cell count,disease severity,ESR,and hs-CRP levels(P<0.05).The levels of serum sE-selectin,IL-36γ,and IL-31 in the subgroup with poor outcomes were higher than those in the subgroup with good outcomes(P<0.05).The AUC value of the combined assessment of serum sE-selectin,IL-36γ,and IL-31 levels for disease outcome was greater than that of single indicator assessment(P<0.05).Conclusion The levels of serum sE-selectin,IL-36γ,and IL-31 in patients with PsV are elevated and positively correlated with the severity of the disease.Combined detection of their levels has certain evaluation value for disease prognosis.
