摘 要:目的分析孤独G蛋白偶联受体配体-13(Apelin-13)、微小RNA-21(miR-21)与急性心肌梗死(AMI)心肌损伤程度及预后情况的关联。方法选取92例AMI患者,将Killip分级为Ⅰ~Ⅱ级的42例患者列为轻度组,将Killip分级为Ⅲ~Ⅳ级的50例患者列为中重度组,比较两组患者的Apelin-13、miR-21以及心肌损伤标志物的差异;分析血清Apelin-13、miR-21与心肌损伤标志物的相关性及对AMI患者主要心血管不良事件(MACE)发生风险的预测价值。结果中重度组的Apelin-13、miR-21相对表达量,以及MYO、cTnT、CK-MB、LDH的水平均高于轻度组,差异具有统计学意义(P<0.05)。血清Apelin-13与MYO、cTnT、CK-MB、LDH的水平均呈正相关(P<0.05);血清miR-21与MYO、cTnT、CK-MB、LDH的水平均呈正相关(P<0.05)。92例AMI患者中MACE发生率为21.74%(20/92)。发生组的Apelin-13、miR-21相对表达量,以及MYO、cTnT、CK-MB、LDH的水平均高于未发生组(P<0.05)。血清Apelin-13对AMI继发MACE的预测灵敏度、特异度分别为82.44%、79.32%,血清miR-21对AMI继发MACE的预测灵敏度、特异度分别为80.77%、77.95%。结论动态监测血清Apelin-13、miR-21水平能实现对患者MACE发生风险的早期预测。Objective To analyze the correlation between serum levels of solitary G protein coupled receptor ligand-13(Apelin-13),microRNA-21(miR-21),and the severity and prognosis of myocardial injury in the patients with acute myocardial infarction(AMI).Methods 92 patients with AMI were selected,and 42 patients with Killip grades I-II were classified as the mild group,while 50 patients with Killip gradesⅢ-Ⅳwere classified as the moderate to severe group.The differences in the serum Apelin-13,the expression levels of miR-21,and the levels of serum myocardial injury markers between the two groups were compared.Pearson correlation coefficient was used to test the correlation between the serum Apelin-13,the expression levels of miR-21 and the levels of serum myocardial injury markers.The predictive value of serum Apelin-13 and the expression levels of miR-21 for the risk of major cardiovascular adverse events(MACE)in AMI patients was calculated.Results The serun levels of Apelin-13,the relative expression levels of miR-21,the levels of serum MYO,cTnT,CK-MB,and LDH in the moderate to severe group were significantly higher than those in the mild group,showing a statistically significant difference(P<0.05).The levels of serum Apelin-13 was positively correlated with the levels of serum MYO,cTnT,CK-MB,and LDH(P<0.05),while the levles of serum miR-21 was positively correlated with the levels of serum MYO,cTnT,CK-MB,and LDH(P<0.05).The incidence of MACE in 92 patients with AMI was 21.74[FGS]%[FGN](20/92).The levels of serum Apelin-13,the relative expression levels of miR-21,the levels of serum MYO,cTnT,CK-MB,and LDH in the occurrence group were higher than those in the non occurrence group(P<0.05).The sensitivity and specificity of serum Apelin-13 levels for predicting MACE secondary to AMI were 82.44[FGS]%[FGN]and 79.32[FGS]%[FGN],respectively.The sensitivity and specificity of serum miR-21 levels for predicting MACE secondary to AMI were 80.77[FGS]%[FGN]and 77.95[FGS]%[FGN],respectively.Conclusion Dynamic monitoring of serum A
