摘　　要：目的研究半乳糖凝集素3(Gal-3)在缺血性心肌病(ICM)心力衰竭(简称心衰)及非缺血性心肌病(NICM)心衰患者中表达的差异,同时研究Gal-3与心功能的关系,评估其在心衰中的诊断价值。方法选取2022年1—10月于郑州大学第二附属医院就诊的心功能Ⅱ~Ⅳ级心衰患者,并将其分为ICM心衰组39例和NICM心衰组36例,另选取同期对照组35例。入院后均采用酶联免疫吸附试验法检测血浆中Gal-3的表达水平,比较各组间Gal-3及相关指标的差异,进行数据统计分析。结果3组患者Gal-3水平差异有统计学意义(P<0.05),且任意两组间差异均有统计学意义(P<0.05)。在ICM心衰组、NICM心衰组中,心功能Ⅱ、Ⅲ级患者关于Gal-3差异均具有统计学意义(P<0.001),且Gal-3在Ⅲ级患者中的水平高于Ⅱ级。ICM心衰组患者Gal-3水平与左心室射血分数(LVEF)存在负相关关系(r=-0.576,P<0.001),与左室舒张末期内径(LVEDD)存在正相关关系(r=0.494,P=0.001)。Gal-3对心衰诊断的受试者工作特征(ROC)曲线:曲线下面积(AUC)为0.851(95%CI:0.778~0.924,P<0.001),最佳阈值为3.68μg·L^(-1),灵敏度73.3%,特异度82.9%。Gal-3对ICM心衰诊断的ROC曲线:AUC为0.898(95%CI:0.837~0.958,P<0.001),最佳阈值为4.505μg·L^(-1),灵敏度74.4%,特异度93.0%。结论Gal-3对心衰患者具有一定的诊断价值,Gal-3对于ICM心衰患者诊断的特异性价值进一步提高,Gal-3与ICM心衰组患者的心功能存在相关性,这些提示其与心肌重构可能相关。Objective To study the difference of galectin-3(Gal-3)expression in ischemic cardiomyopathy(ICM)heart failure and non-ischemic cardiomyopathy(NICM)heart failure patients,and to study its correlation with cardiac function and evaluate its clinical value.Methods Patients with gradeⅡ-Ⅳheart failure with cardiac function admitted to the Second Affiliated Hospital of Zhengzhou University from January to October 2022 were selected and divided into ICM heart failure group(39 cases)and NICM heart failure group(36 cases).The corresponding control group(35 cases)was also selected.After admission,the expression level of Gal-3 in plasma was detected by enzyme-linked immunosorbent assay.The differences of Gal-3 and related indexes among all groups were compared,and the data were statistically analyzed.Results The difference of Gal-3 level among the three groups was statistically significant(P<0.05),and the difference between any two groups was statistically significant(P<0.05).In the ICM and NICM heart failure groups,there were statistically significant differences in Gal-3 in gradeⅡandⅢheart function patients(P<0.001),and the level of Gal-3 in gradeⅢwas higher than that in gradeⅡ.Gal-3 level was negatively correlated with left ventricular ejection fraction(LVEF)(r=-0.576,P<0.001)and positively correlated with left ventricular end diastolic diameter(LVEDD)(r=0.494,P=0.001)in ICM group.The receiver operating characteristic(ROC)curve area under(AUC)of Gal-3 for diagnosis of heart failure was 0.851(95%CI:0.778-0.924,P<0.001),the optimal threshold was 3.68μg·L^(-1),the sensitivity was 73.3%,and the specificity was 82.9%.The AUC of Gal-3 for ICM heart failure diagnosis was 0.898(95%CI:0.837-0.958,P<0.001),the optimal threshold was 4.505μg·L^(-1),the sensitivity was 74.4%,and the specificity was 93.0%.Conclusion Gal-3 has certain diagnostic value for patients with heart failure.The specificity value of Gal-3 in the diagnosis of ICM heart failure patients has further improved.There is a correlation between Gal-3 and
