摘  要:目的探讨振幅整合脑电图(aEEG)联合新生儿行为神经测评(NBNA)量表、全身运动(GMs)质量评估与早产儿脑损伤的关系。方法选取2021年4月至2023年3月郑州市妇幼保健院分娩的104例早产儿作为研究对象,根据专家共识分为脑损伤组(48例)和非脑损伤组(56例)。两组早产儿均于出生后12 h内进行aEEG检测;孕周纠正至40周时,于出生后3 d和出生后14 d进行NBNA检查;于纠正胎龄1个月和3个月时进行扭动运动和不安运动阶段GMs质量评估。结果脑损伤组患儿aEEG评分以及出生后3 d、出生后14 d时的NBNA评分均低于非脑损伤组,扭动运动和不安运动阶段GMs质量评估异常占比均高于非脑损伤组,差异有统计学意义(P<0.05)。logistic回归分析显示,早产儿脑损伤的发生与aEEG评分(OR=0.324,95%CI:0.109~0.965)、出生14 d时NBNA评分(OR=0.019,95%CI:0.002~0.213)以及扭动运动阶段GMs异常(OR=4.563,95%CI:1.159~17.958)等指标存在密切关系(P<0.05)。结论aEEG监测、NBNA量表和GMs质量在早产儿脑损伤中具有一定意义,其中aEEG评分、出生后14 d时NBNA评分以及扭动运动阶段GMs质量评估3种指标与早产儿脑损伤密切相关。Objective To investigate the relationship of amplitude-integrated electroencephalogram(aEEG)combined with neonatal behavioral neurological assessment(NBNA)and general movements(GMs)quality assessment in premature infants with brain injury.Methods A total of 104 premature infants delivered Zhengzhou Maternal and Child Health Care Hospital from April 2021 to March 2023 were selected as the study objects,and were divided into brain injury group(48 cases)and non-brain injury group(56 cases)according to expert consensus.aEEG was detected within 12 hours after birth in both groups.When the gestational age was corrected to 40 weeks,NBNA was tested at 3 and 14 days after birth.GMs quality in the torsion and restless movement stages was assessed at 1 and 3 months of corrected gestational age.Results The aEEG scores and NBNA scores at 3 and 14 days after birth in the brain injury group were lower than those in the non-brain injury group,and the abnormal proportion of GMs quality assessment in the torsion and restless movement stages was higher than that in the non-brain injury group(P<0.05).Logistic regression analysis was showed that,the occurrence of brain injury in preterm infants was associated with aEEG score(OR=0.324,95%CI:0.109-0.965),NBNA score at 14 days after birth(OR=0.019,95%CI:0.002-0.213)and abnormal GMs in the torsion movement(OR=4.563,95%CI:1.159-17.958)(P<0.05).Conclusion aEEG monitoring,NBNA scale and GMs quality assessment were of certain significance of premature infants with brain injury,and the three indicators of aEEG score,NBNA score at 14 days after birth and GMs quality assessment in the torsion stage were closely related to premature infants with brain injury.
