摘  要:目的探讨基于CT血管造影(CTA)血管定位与三维重建辅助设计的股前外侧皮瓣修复口腔癌术后软组织缺损的效果。方法选取2021年1月至2023年10月在郑州大学第一附属医院治疗的口腔癌术后软组织缺损患者92例,采用信封法将患者分为观察组(46例)和对照组(46例),两组均接受股前外侧皮瓣修复,其中观察组接受基于CTA血管定位与三维重建辅助设计,对照组接受彩超定位。结果观察组手术时间、术中出血量、穿支动脉内径术前评估与实际差异、制备皮瓣时间低于对照组(P<0.05),穿支点符合率高于对照组(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论基于CTA血管定位与三维重建辅助设计的股前外侧皮瓣修复口腔癌术后软组织缺损有较好的效果,具有皮瓣制备时间短、术中出血少、并发症少等优点。Objective To explore effect of anterolateral thigh flap designed based on CT angiography(CTA)vascular localization and 3D reconstruction in repairing the soft tissue defect after oral cancer surgery.Methods A total of 92 patients with postoperative soft tissue defect of oral cancer treated in the First Affiliated Hospital of Zhengzhou University from January 2021 to October 2023 were selected,divided the patients into observation group(46 cases)and control group(46 cases)by envelope method.Both groups received femoral anterolateral flap repair,and the observation group received auxiliary design based on CTA vascular localization and 3D reconstruction,and the control group received color ultrasound positioning.Results Surgical time,intraoperative blood loss,difference between preoperative evaluation and actual diameter of the perforating artery,and flap preparation time in the observation group were lower than that in the control group(P<0.05),the compliance rate of the puncture point was higher than that of the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion The anterior lateral thigh flap based on CTA vascular localization and 3D reconstruction assisted design has good results in repairing soft tissue defects after oral cancer surgery,with advantages such as short flap preparation time and less intraoperative bleeding.
