自拟参苓白术固本汤联合维生素D治疗小儿反复呼吸道感染的效果  

Effect of Self-made Shenling Baizhu Guben Decoction Combined with Vitamin D in the Treatment of Children with Recurrent Respiratory Tract Infection

摘　　要：目的探讨自拟参苓白术固本汤联合维生素D治疗小儿反复呼吸道感染(RRTI)的临床效果。方法使用随机数表法将2022年1月至2024年1月在洛阳市妇幼保健院就诊的76例RRTI患儿均分为两组。对照组接受常规治疗和维生素D治疗,在对照组基础上给予观察组患儿自拟参苓白术固本汤治疗。比较两组患儿症状缓解时间、炎症因子[半胱氨酰白三烯(CysLTs)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、T细胞亚群(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))水平以及呼吸道感染再发率。结果观察组患儿各临床症状消失时间均较早(P<0.05);治疗后,两组CysLTs、IL-6、TNF-α水平均下降,而CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均上升,且治疗后观察组CysLTs、IL-6、TNF-α较低,而CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平较高(P<0.05);两组呼吸道感染再发率(21.05%比5.26%)比较,差异有统计学意义(P<0.05)。结论自拟参苓白术固本汤联合维生素D治疗可有效改善RRTI患儿炎症反应和免疫功能,对加快患儿康复,减少呼吸道感染再发有积极帮助。Objective To investigate the clinical efficacy of self-made Shenling Baizhu Guben Decoction combined with vitamin D in the treatment of recurrent respiratory tract infection(RRTI)in children.Methods A total of 76 children with RRTI who were treated in Luoyang Maternal and Child Health Hospital from January 2022 to January 2024 were divided into observation group and control group using a random number table.The control group received conventional treatment and vitamin D,the children in the observation group were treated with self-made Shenling Baizhu Guben Decoction on the basis of the control group.The symptom relief time,inflammatory factors[cysteinyl leukotriene(CysLTs),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],T cell subsets(CD3^(+),CD4^(+),CD4^(+)/CD8^(+))and respiratory tract infection recurrence rate were compared between the two groups.Results The disappearance time in the observation group was earlier(P<0.05).After treatment,the levels of CysLTs,IL-6 and TNF-αin the two groups decreased,while the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)increased,and the levels of CysLTs,IL-6 and TNF-αin the observation group were lower,while the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were higher(P<0.05).The recurrence rate of respiratory infection in the two groups(21.05%vs 5.26%)were significantly different(P<0.05).Conclusion Self-made Shenling Baizhu Guben Decoction combined with vitamin D can effectively improve the inflammatory response and immune function of children with RRTI,and can accelerate the recovery of children,and reduce the recurrence of respiratory tract infection.
