摘　　要：目的观察针灸配合蒲藤化瘀消炎汤保留灌肠对输卵管梗阻性不孕患者盆腔局部微循环的影响。方法本研究将2021年6月至2022年9月就诊于河南中医药大学第三附属妇产科的78例输卵管梗阻性不孕患者,以随机数字表分为对照组(39例接受输卵管通液术治疗,术后口服阿奇霉素片治疗)和观察组(39例在对照组基础上接受针灸配合蒲藤化瘀消炎汤保留灌肠治疗)。统计两组3个周期后输卵管再通率,评估两组盆腔局部微循环指标及中医症状评分变化。随访1 a,记录两组妊娠和复发情况。结果观察组输卵管再通率高于对照组,差异有统计学意义(P<0.05)。两组3个周期后血流指数(FI)高于治疗前,观察组高于对照组(P<0.05);两组3个周期后搏动指数(PI)、阻力指数(RI)低于治疗前,观察组低于对照组(P<0.05)。两组治疗前中医症状评分比较,差异无统计学意义(P>0.05)。两组3个周期后下腹疼痛、经色紫黯、面色晦暗、神疲乏力、情志抑郁评分及总评分低于治疗前,观察组低于对照组(P<0.05)。随访1 a,观察组妊娠率高于对照组,复发率低于对照组,差异有统计学意义(P<0.05)。结论针灸配合蒲藤化瘀消炎汤保留灌肠可改善输卵管梗阻性不孕患者盆腔局部微循环,提高妊娠率,降低疾病的复发。Objective To observe the effects of acupuncture and moxibustion combined with Puteng Huayu Xiaoyan Decoction retention enema on local pelvic microcirculation in patients with tubal obstructive infertility.Methods In this study,78 patients with tubal obstruction infertility who attended the Department of Obstetrics and Gynaecology of the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from June 2021 to September 2022 were divided into the control group(39 cases received tubal ligation treatment,and oral azithromycin tablets after the operation)and the observation group(39 cases received acupuncture combined with Puteng Huayu Xiaoyan Decoction retention enema on the basis of control group)with the help of a random number table.The rate of tubal recanalisation after 3 cycles was counted in both groups,and changes in local pelvic microcirculation indexes and traditional Chinese medicine symptom scores were assessed in both groups.The pregnancy and recurrence were recorded in both groups after 1 year of follow-up.Results The rate of tubal recanalisation in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Flow index(FI)after 3 cycles was higher than that before treatment in both groups,and the observation group was higher than that in the control group(P<0.05).Pulsation index(PI)and resistance index(RI)were lower than that before treatment in both groups after 3 cycles,and the observation group were lower than that in the control group(P<0.05).Comparison of traditional Chinese medicine symptom scores before treatment between the two groups,the difference was not statistically significant(P>0.05).The scores of lower abdominal pain,purplish colour of menstruation,dark complexion,fatigue,emotional depression and total scores were lower than those before treatment in both groups after 3 cycles,and the observation group were lower than those in the control group(P<0.05).After 1 year of follow-up,the pregnancy rate of the ob
