消颤调肝汤治疗帕金森病合并轻度认知障碍的效果

摘　　要：目的探讨消颤调肝汤治疗帕金森病(PD)合并轻度认知障碍的效果。方法选择许昌市立医院2020年6月至2023年2月收治98例PD患者进行前瞻性随机对照研究,通过随机数字表法将其分为对照组与观察组,各49例。对照组接受一般治疗(营养支持、心理支持、生活支持)联合药物(多巴丝肼+卡巴拉汀)治疗,另给予认知训练、经颅直流电刺激、有氧运动等非药物干预;观察组在对照组基础上加用消颤调肝汤,两组均连续治疗3个月。对比治疗前、治疗3个月时疾病恢复情况[统一帕金森病评定量表(UPDRS)]、认知功能[蒙特利尔认知评估量表(MoCA)、帕金森病认知评定量表(PD-CRS)]、不良反应(恶心、呕吐、直立性低血压、排尿困难)。结果治疗后,两组精神与情感障碍、日常生活能力、运动体征评分均下降,且观察组低于对照组(P<0.05)。治疗后,两组MoCA、PD-CRS评分均上升,且观察组高于对照组(P<0.05)。两组不良反应总发生率对比,差异无统计学意义(P>0.05)。结论消颤调肝汤可辅助西药进一步促进PD患者疾病恢复,且可改善轻度认知障碍,效果较好。Objective To explore the therapeutic effect of Xiaochan Tiaogan Decoction on Parkinson’s disease(PD)with mild cognitive impairment.Methods A total of 98 PD patients in Xuchang Municipal Hospital from June 2020 to February 2023 were selected for a prospective randomized controlled study.They were divided into a control group and an observation group by random number table method,with 49 cases in each group.The control group received general treatment(nutritional support,psychological support,life support)combined with drugs(benserazide and rivastigmine).The observation group was supplemented with Xiaochan Tiaogan Decoction on the basis of the control group,and both groups were treated continuously for 3 months.The disease recovery[unified Parkinson’s disease rating scale(UPDRS)],cognitive function[Montreal cognitive assessment(MoCA),Parkinson’s disease cognitive rating scale(PD-CRS)],and adverse reactions(nausea,vomiting,orthostatic hypotension,dysuria)were compared before treatment and 3 months after treatment.Results After treatment,the scores of mental and emotional disorders,daily living ability,and physical signs in both groups decreased,and the observation group was lower than the control group(P<0.05).After treatment,the MoCA and PD-CRS scores of both groups increased,and the observation group was higher than the control group(P<0.05).There was no statistical significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Xiaochan Tiaogan Decoction can assist western medicine to further promote the recovery of PD patients,and can improve mild cognitive impairment,with good effect.
