摘 要:目的评估3D-优化采集可变翻转角快速自旋回波(SPACE)序列和2D-T_(2)加权半傅里叶采集快速自旋回波(HASTE)序列对儿童腰椎脊髓圆锥及终丝显示清晰度的效能比较。方法回顾性分析2023年11月至2024年5月,深圳市儿童医院中出现临床症状且经临床诊断后高度怀疑患有隐匿性脊髓栓系综合征的50例患者。利用西门子Prisma 3.0 T磁共振成像系统采集患儿术前的腰椎图像,所有患儿均行俯卧位3D-SPACE序列和2D-HASTE序列扫描。2位10年以上工作经验的儿童神经放射医生,分别使用4分法对腰椎脊髓圆锥及终丝显示情况进行分级评分。使用Mann-Whitney U检验进行统计分析,比较2组序列评分。采用加权Kappa检验评价2名医师间主观评分的一致性,根据加权Kappa值判断一致性强度。结果脊髓圆锥和终丝3D-SPACE序列图像清晰度的评分均高于2D-HASTE序列,差异具有统计学意义(P均<0.001)。HASTE序列的脊髓终丝评分一致性较强(加权Kappa=0.716)、脊髓圆锥评分一致性强(加权Kappa=0.807);SPACE序列的脊髓圆锥和终丝评分一致性均强(加权Kappa=0.827,0.831)。结论在儿童腰椎脊髓俯卧位MR成像中,3D-SPACE序列脊髓圆锥及终丝显示优于2D-HASTE序列,值得临床推广使用。Objective To evaluate the effectiveness of the 3D-SPACE sequence and the 2D-HASTE sequence in the clarity of displaying the conus medullaris and filum terminale in pediatric lumbar spinal cord magnetic resonance imaging(MRI).Methods A retrospective analysis was conducted on 50 patients in Shenzhen Children′s Hospital from November 2023 to May 2024.These patients had clinical symptoms and were highly suspected of having occult tethered spinal cord syndrome after clinical diagnosis.Siemens Prisma 3.0 T magnetic resonance imaging(MRI)system was used to collect preoperative lumbosacral and coccygeal images of children.All children underwent prone 3D sampling perfection with application optimized contrast using different flip angle evolution(3D-SPACE)sequence and 2D half-fourier acquisition single-shot turbo spin-echo(2D-HASTE)sequence scanning.Two pediatric neur-oradiologists with more than 10 years of work experience independently graded the display of the lumbar spinal cord conus medullaris and filum terminale using a four-point scale.The Mann-Whitney U test was used for statistical analysis to compare the scores of the two sequences.Weighted Kappa test was used to evaluate the consistency of subjective scoring between the two physicians,and the strength of consistency was judged according to the weighted Kappa value.Results The clarity scores of the conus medullaris and filum terminale images in the 3D-SPACE sequence were all higher than those in the 2D-HASTE sequence,and the differences were statis-tically significant(P all<0.001).The consistency of the filum terminale scoring in the HASTE sequence was strong(weighted Kappa=0.716),and the conus medullaris scoring was also strong(weighted Kappa=0.807)the consistency of both the conus medullaris and filum terminale scoring in the SPACE sequence was very strong(weighted Kappa=0.827,0.831).Conclusion In pediatric lumbar spinal MRI with a prone position,the 3D-SPACE sequence demonstrates superior visualization of the conus medullaris and filum terminale compared to
