摘 要:目的探讨超声心动图在川崎病患儿冠状动脉病变评估中的应用价值,通过比较川崎病患儿与健康对照组的冠状动脉内径变化、动脉瘤形成率及血流异常发生率,评估川崎病对冠状动脉功能的影响。方法选择2020年1月至2023年12月在我院确诊的46例川崎病患儿作为观察组,另选择同期46例健康幼儿作为对照组。通过超声心动图监测2组患者在发病后3~7天、20~28天及80~117天的冠状动脉内径、动脉瘤形成及血流异常发生情况。采用χ^(2)检验和t检验对数据进行统计分析。结果观察组患儿在随访期间冠状动脉内径显著增大,内径变化率为19.62%,远高于对照组的2.46%(χ^(2)=4.723,P<0.001)。观察组18例(39.13%)患儿形成冠状动脉动脉瘤,显著高于对照组2例(4.35%)(χ^(2)=46.290,P<0.001)。冠状动脉血流异常发生率在观察组也明显高于对照组,在第7天、28天和117天的随访中,血流异常发生率分别为16例(34.78%)、18例(39.13%)和14例(30.43%),而对照组分别为2例(4.35%)、3例(6.52%)和4例(8.70%)(P<0.001)。结论超声心动图是评估川崎病患儿冠状动脉病变的有效工具。通过对冠状动脉内径、动脉瘤形成率和血流异常的监测,能够及早发现川崎病患儿的冠状动脉病变,并为临床决策提供重要依据。Objective To investigate the value of echocardiography in the assessment of coronary artery lesions in children with Kawasaki disease,and to assess the impact of Kawasaki disease on coronary artery function by comparing the changes in coronary artery internal diameter,the rate of aneurysm formation,and the incidence of blood flow abnormalities between children with Kawasaki disease and the healthy control group.Methods Forty-six children with Kawasaki disease diagnosed in our hospital from January 2020 to December 2023 were selected as the observation group,and 46 healthy young children in the same period were selected as the control group.The occurrence of intracoronary artery diameter,aneurysm formation and blood flow abnormalities in both groups were monitored by echocardiography at 3-7,20-28 and 80-117 days after the onset of the disease.The data were statistically analyzed using theχ^(2) test and t-test.Results The inner diameter of the coronary arteries of the children in the observation group increased significantly during the follow-up period,and the rate of change in inner diameter was 19.62%,which was much higher than that of the control group,which was 2.46%(χ^(2)=4.723,P<0.001).Coronary artery aneurysm was formed in 18(39.13%)children in the observation group,which was significantly higher than in 2(4.35%)children in the control group(χ^(2)=46.290,P<0.001).The incidence of coronary artery flow abnormalities was also significantly higher in the observation group than in the control group,with 16(34.78%),18(39.13%)and 14(30.43%)cases at day 7,28 and 117 days of follow-up,respectively,as compared to 2(4.35%),3(6.52%)and 4(8.70%)cases,respectively,in the control group(P<0.001).Conclusion Echocardiography is an effective tool to assess coronary artery lesions in children with Kawasaki disease.By monitoring the internal diameter of the coronary arteries,the rate of aneurysm formation,and blood flow abnormalities,coronary artery lesions in children with Kawasaki disease can be detected at an early stage a
